周围型肺错构瘤MSCT表现

MSCT features of peripheral pulmonary hamartoma

目的探讨周围型肺错构瘤的MSCT特点,以提高对本病的诊断符合率。方法回顾性分析26例经手术病理证实的周围型肺错构瘤患者的MSCT表现。结果本组26例均为周围型单发肺错构瘤,其中位于右肺11例,左肺15例。病灶呈椭圆形12例,圆形9例,不规则形5例。病灶见有浅分叶10例,深分叶5例,无分叶11例。病灶内无脂肪密度和钙化组织,密度均匀14例,平均CT值(14.9±24.2);病灶内含有不同程度脂肪组织,密度不均匀12例,平均CT值(19.6±20.1)HU。病灶内伴有钙化11例,其中呈典型爆米花样钙化4例,斑点状钙化6例,环状钙化1例。病灶周边见有血管集束征9例,邻近胸膜牵线征4例。增强19例均呈轻度强化,动脉期平均CT值(24.6±12.0)HU;静脉期平均CT值(34.8±11.5)HU;平衡期平均CT值(33.2±9.2)HU。结论瘤内伴脂肪和钙化是周围型肺错构瘤的典型征象,边界清晰及增强见轻度强化是重要依据,病灶分叶及胸膜牵线征不能除外肺错构瘤可能。

Objective To investigate the characteristics of peripheral pulmonary hamartoma with MSCT,in order to improve the accuracy of diagnosis of the disease.Methods The MSCT manifestations in 26 peripheral pulmonary hamartoma patients who were confirmed by pathology were retrospectively analyzed.Results All 26 cases were peripheral solitary pulmonary hamartoma,of which 11 cases were located in the right lung and 15 cases in the left lung.The lesions were oval in 12 cases,round in 9 cases and irregular in 5 cases.There were 10 cases of superficial lobule,5 cases of deep lobule,and 11 cases without lobule.There was no fat density and calcified tissue in the lesion,the density was homogeneous in 14 cases and the mean CT value(14.9±24.2);There were different degrees of adipose tissue in the lesion,12 cases of uneven density,and the average CT value(19.6±20.1)HU.Calcification was found in 11 cases,including 4 cases of typical popcorn calcification,6 cases of spotted calcification and 1 case of calcification.There were 9 cases of vascular bundle around the lesion and 4 cases of adjacent pleural pull sign.All 19 cases showed mild enhancement,the mean CT value(24.6±12)HU in arterial phase,mean CT value(34.8±11.5)HU in venous phase,and mean CT value(33.2±9.2)HU in balance phase.Conclusion Fat and calcification are the typical signs of peripheral pulmonary hamartoma.Clearboundary and no enhancement or slight enhancement is also the important diagnostic basis,and lobulation and pleural up syndrome do not exclude the possible pulmonary hamartoma.
