联合ADC值与最大强化百分比提高卵巢交界性囊腺瘤的诊断效能 被引量:7

摘要 目的探讨磁共振(MRI)弥散加权成像(DWI)参数表观弥散系数(ADC)值与增强序列病灶信号最大强化百分比(MEP)联合应用在提高卵巢交界性囊腺瘤诊断效能方面的价值。方法回顾分析本院经手术病理证实具有实性成分或囊壁/囊隔增厚的23例卵巢良性囊腺瘤(A组)、17例卵巢交界性囊腺瘤(B组)和25例卵巢囊腺癌(C组)患者的MRI资料。观察病灶DWI及增强序列信号特征。测量病灶实性部分或囊壁/囊隔的ADC值和MEP,比较组间的差异。使用二项Logistic回归结合受试者工作特征(ROC)曲线分析ADC值、MEP以及两者联合(ADC+MEP)在组间的诊断效能并使用MedCalc软件进行统计学比较。结果病灶实性部分或囊壁/囊隔ADC值从A组至C组逐渐减低,两两比较均具有统计学差异(P均<0.05)。MEP从A组至C组逐渐增高,然而仅在A-C、B-C组间存在统计学差异(P均<0.05)。ADC值鉴别A-B两组病灶的曲线下面积(AUC)为0.813。ADC值、MEP以及ADC+MEP鉴别B-C两组病灶的AUC分别为0.861、0.860以及0.941,AUCADC+MEP与AUCADC、AUCMEP之间均存在统计学差异(P均<0.05)。结论结合MRI增强MEP和量化的ADC值有利于提高交界性囊腺瘤的诊断效能,为临床选择合适的治疗方案奠定基础。
