血NF-KB P65表达在创伤性湿肺并ARDS患者凝血功能中的意义及护理对策

Significance of Blood NF-KB P65 Expression in Coagulation Function of Patients with Traumatic Wet Lung and ARDS and Nursing Countermeasures

目的 分析血NF-KBP65表达在创伤性湿肺并急性呼吸窘迫综合征(ARDS)患者凝血功能的意义,总结护理经验。方法 选择2017年2月~2018年4月我院收治的46例创伤性湿肺并ARDS患者纳入观察组。选择同期收治的46例创伤性湿肺住院患者纳入对照组。入院时均检测凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)、血NF-KBP65表达水平,将观察组按照预后进行分组,其中死亡、严重并发症纳入不良组,其余患者纳入良好组。并对凝血功能指标与血NF-KBP65表达进行相关性分析。结果 观察组入院时PT、APTT、TT、FIB、血NF-KBP65表达水平高于对照组,差异有统计学意义(P<0.05)。观察组入院时PT、APTT、TT、FIB与血NF-KBP65表达存在相关性,差异有统计学意义(P<0.05)。良好组PT、APTT、TT、FIB、血NF-KBP65表达水平低于不良组,差异有统计学意义(P<0.05)。结论 对于创伤性湿肺并ARDS患者,需要重视病情及预后观察、指标监测,对于预后不良高风险的患者,需采取积极的护理干预。

Objective To analyze the significance of blood NF-KB P65 expression in coagulation function of patients with traumatic wet lung and acute respiratory distress syndrome, and summarize nursing experience. Methods From February 2017 to April 2018, 46 patients with Traumatic wet lung and ARDS were admitted into the observation group, and 46 patients with traumatic wet lung admitted in the same period were selected as the control group. The levels of prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), fibrinogen (FIB) and blood NF-KB P65 were detected at admission. The observation group was grouped according to the prognosis. Death and serious complications were included in the bad group, while the rest were included in the good group. Coagulation function index and blood NF-KB P65 correlation was analyzed. Results PT, APTT, TT, FIB and blood NF-KB P65 were higher in the observation group than in the control group at admission, and the difference was statistically significant (P<0.05). The PT, APTT, TT, FIB were correlated with blood NF-KB P65 at admission in the observation group, and the difference was statistically significant (P<0.05). PT, APTT, TT, FIB and blood NF-KB P65 in the good group were lower than those in the bad group (P<0.05). Conclusion For patients with traumatic wet lung and ARDS, we need to pay attention to the condition and prognosis observation, indicators monitoring. For those with poor prognosis and high risk, we need to take active nursing intervention.
