健康控制点对造口还纳术后并发症疗效及Rosenberg量表评分的影响研究

Effect of health control point on postoperative complications after ostomy and Rosenberg scale

目的探讨健康控制点对造口还纳术后患者临床并发症的疗效及对Rosenberg自尊量表评分的影响。方法选取华中科技大学同济医学院附属武汉市中心医院胃肠外科2017年6月一2018年12月102例行肠道造口还纳术的患者,采用随机表法分为健康控制组和对照组各51例,对照组采用常规护理,健康控制组在对照组基础上行健康控制点管理,对比2组患者并发症发生情况和Rosenberg自尊量表评分。结果治疗后健康控制组患者3个月内并发症总发生率[3.9%(2/51)]低于对照组[15.7%(8/51)],差异具有统计学意义(U=1.98,P=0.048);治疗前,2组患者评分差异无统计学意义(t=0.635,P=0.527),治疗后,2组患者评分均较前增加,差异具有统计学意义(健康组t=29.031,P=0.000;对照组t=17.909,P=0.000),健康控制组明显高于对照组,差异具有统计学意义(t=9.756,P=0.000)。结论健康控制点管理可有效降低造口还纳术后患者临床并发症发生率,提高患者自尊心,值得临床应用推广。

Objective To investigate the effect of health control points on clinical complications and the score of Rosenberg′s self-esteem scale in patients after the reconstruction of stoma. Methods The patients selected from June 2017 to December 2018 in this hospital were divided into 51 cases in the health control group and 51 cases in the control group. The control group used routine care and the health control group managed by the health control points on the basis of the control group. The complications and Rosenberg′s self-esteem scale were compared between the two groups. Results The total incidence of complications in Post-treatment health control group within three months was lower than that in control group [3.9%(2/51) vs 15.7%(8/51)], and the difference was statistically significant(U=1.98, P=0.048);Before the treatment, there was no significant statistical difference between the two groups(t=0.635, P=0.527). After the treatment, the scores of the two groups increased compared with the previous ones, and the difference was statistically significant(health group t=29.031, P<0.001;The control group t=17.909, P<0.001), the health control group was significantly higher than the control group, and the difference was statistically significant(t=9.756, P<0.001). Conclusion Health control point management can effectively reduce the incidence of clinical complications in postoperative patients after the reconstruction of stoma, and improve patients′ self-esteem, which is worthy of clinical application and promotion.
