混合痔患者术后两种不同熏蒸液坐浴的疗效观察

Therapeutic Effect Observation of Two Different Fumigation Baths in Patients with Mixed Hemorrhoids after Operation

目的为痔疮患者术后预防肛门术区感染及肛缘水肿的发生,选择最佳的护理方法。方法随机选取我院普外肛门科2018年2月至2019年2月诊断为“混合痔Ⅲ度”并行手术治疗的患者100例,随机分成对照组和观察组,每组各50例。100例患者术后住院期间给予常规护理,对照组患者每次排便后用1∶5000高锰酸钾溶液熏蒸坐浴,每日常规用1∶5000高锰酸钾溶液熏蒸坐浴2次;观察组将1∶5000高锰酸钾溶液替换成肛门冲洗剂进行熏蒸坐浴,比较两组患者术后第7天发生肛门术区感染的例数及发生肛缘水肿的例数。结果观察组术后第7天肛门术区感染率(4%)低于对照组术后第7天肛门术区感染率(16%),差异有统计学意义(P<0.05);观察组术后第7天肛缘水肿发生率(14%)低于对照组术后第7天肛缘水肿发生率(34%),差异有统计学意义(P<0.05)。结论肛门冲洗剂可以降低患者术后肛门术区感染率及降低患者术后肛缘水肿发生率。

Objective To select the best nursing method to prevent the patients from postoperative anal infection and anal edema. Methods From February 2018 to February 2019,100 patients in General Anal Department of our hospital diagnosed with “mixed hemorrhoids of degreeⅢ”were randomly divided into two groups: control group ( n =50) and observation group ( n =50). Routine nursing was given to 100 patients during postoperative hospital stay. The control group was fumigated with 1∶5000 potassium permanganate solution in a sitz bath after each bowel movement twice a day;the observation group replaced 1∶5000 potassium permanganate solution with an anal irrigation for fumigation bathing,and compared the number of cases of anal surgery infection and the number of cases of anal edema on the 7th day after operation in these two groups. Results The infection rate of anal operation area in observation group (4%) on the 7th day after operation was lower than that in control group (16%) on the 7th day after operation,and the difference was statistically significant ( P <0.05). The incidence of anal edema (14%) was lower than that of the control group on the 7th day after operation (34%),and the difference was statistically significant ( P <0.05). Conclusion Anal irrigation can reduce the infection rate of postoperative anal region and decrease the incidence of postoperative anal edema.
