时机理论下的延续护理在肺癌患者中的应用

Application of a continuity-of-care based on timing theory in lung cancer patients

目的探讨时机理论指导下的延续护理对肺癌患者自我护理能力、生活质量及家庭照顾者照顾能力的影响。方法将120例患者及家庭照顾者随机分为干预组60例和对照组60例。干预组在常规护理基础上给予以时机理论为指导的延续护理干预,对照组接受肿瘤科常规随访及护理。分别于干预前、干预后6个月应用家庭照顾者照顾能力问卷(FCTI)、肺癌患者生活质量测量问卷及自我护理能力测定问卷对干预前后患者家庭照顾者照顾能力及患者生活质量、自我护理能力进行评价。结果干预前两组患者家庭照顾者照顾能力及患者生活质量、自我护理能力得分差异均无统计学意义(P>0.05)。干预6个月后,干预组患者家庭照顾者的照顾能力得分低于对照组(P<0.05);两组患者在生活质量及自我护理能力得分高于对照组(P<0.05)。结论以时机理论为指导的延续护理能有效提升家庭照顾者的照顾能力,改善患者生活质量,提高其自我护理能力,值得临床进一步推广应用。

Objective To explore the effects of a continuity-of-care which based on timing theory to patients with lung cancer with their self-care ability,quality of life and caring ability of family caregivers.Methods The 120 patients and family caregivers were randomly divided into intervention group(n=60)and control group(n=60).The intervention group was given a continuity-of-care intervention guided by timing theory on the basis of routine nursing,while control group received routine follow-up and nursing care in Oneology department.The Family Caregiver Care Ability Questionnaire(FCTI),the Lung Cancer Patient Quality of Life Measurement Questionnaire and the Self-care Capacity Measurement Questionnaire were used to evaluate the care ability of the family caregivers and the quality of life and self-care ability of the patients before and after the intervention.Results There were no significant difference in the care ability of the family caregivers and the quality of life and self-care ability scores between the two groups before intervention(P>0.05).After 6 months of intervention,the scores of caring ability of family caregivers in intervention group were lower than those in control group(P<0.05).The scores of quality of life and self-care ability of the two groups were higher than those of control group(P<0.05).Conclusion A continuity-of-care guided by timing theory can effectively improve the care capacity of family caregivers,improve the quality of life and self-care ability of patients.This theory is worthy of further clinical application.
