Ki-67及VEGF在甲状腺乳头状癌中的表达及其临床病理学意义 被引量:9

Expression of Ki-67 and Vascular Endothelial Growth Factor in Papillary Thyroid Carcinoma and Its Clinicopathological Significance

摘要 目的研究甲状腺乳头状癌组织中增殖细胞核抗原( proliferating cell nuclear antigen,PCNA) Ki-67、血管内皮生长因子( vascular endothelial growth factor,VEGF)在甲状腺乳头状癌中的表达,及其与临床病理特征的关系。方法采用回顾性病例对照研究,选取在深圳大学第一附属医院手术治疗的 139 例甲状腺乳头状癌患者的石蜡病理切片,分别对癌组织和癌旁组织进行 Ki-67 及 VEGF 的免疫组化染色,并分析其与临床病理特征之间的关系。结果癌组织中 Ki-67 及 VEGF 阳性表达率分别为 74.82%和 78.42%,高于癌旁组织的 0 和 13.67%,差异具有统计学意义( P= 0.000)。Ki-67、VEGF 的表达与甲状腺乳头状癌患者的年龄、性别、肿瘤大小无显著相关( P>0.05),但与 TNM 分期及有无淋巴结转移有关,在 TNM 分期高的和有淋巴结转移的病例中 Ki-67 和 VEGF 阳性表达率均较高( P<0.05)。Ki-67及 VEGF 共同表达率达 77. 98%,两者明显相关( P< 0. 05)。结论甲状腺乳头状癌组织中 Ki-67 及VEGF 阳性表达率高,与癌旁组织比较有明显差异,与甲状腺乳头状癌的生物学行为密切相关。 Objective To study the expression of proliferating cell nuclear antigen ( PCNA) Ki-67 and vascular endothelial growth factor ( VEGF) in papillary thyroid carcinoma and their relationship with clinicopathological characteristics. Methods A retrospective case-control study was conducted. 139 cases of thyroid papillary carcinoma treated surgically in the First Affiliated Hospital of Shenzhen University were selected for paraffin-embedded pathological sections. Ki-67 and vascular endothelial growth factor immunohistochemical staining were performed on the cancer tissues and adjacent tissues respectively,and the relationship between the indicators and clinicopathological characteristics was analyzed. Results The positive rates of Ki-67 and VEGF in cancer tissues were 74.82% and 78.42% respectively which were higher than that of adjacent tissues ( 0 and 13.67%,P= 0.000). The expression of Ki-67 and VEGF was not significantly correlated with age, gender and size of thyroid papillary carcinoma ( P>0.05),but was correlated with TNM stage and lymph node metastasis ( P< 0. 05). The positive expression rates of Ki-67 and VEGF were higher in high TNM stage and lymph node metastasis patients ( P<0.05). The co-expression rate of Ki-67 and VEGF was 77.98% with a significant correlation ( P<0.05). Conclusion Ki-67 and vascular endothelial growth factor are highly ex- pressed in papillary thyroid cancer tissues,which are significantly different from the adjacent tissues and closely related to the biological behavior of papillary thyroid cancer.
