ERAS理念对糖尿病并结肠癌围手术期并发症影响

EFFECT OF ERAS ON PERIOPERATIVE COMPLICATIONS OF COLORECTAL CANCER IN DIABETIC PATIENTS

目的观察加速康复外科(ERAS)理念对糖尿病并结肠癌病人围手术期并发症的影响,明确ERAS理念对糖尿病人群的安全性与有效性,促进ERAS理念的完善与推广。方法选择糖尿病并结肠癌手术病人97例,随机分为对照组48例和试验组49例,试验组病人应用ERAS理念指导围手术期处理,对照组进行常规处理。结果本研究有86例病人最终完成研究(试验组42例,对照组44例)。结果表明,试验组病人术后排气时间和出院时间较对照组明显缩短,住院费用较对照组明显减少(t=4.218~7.621,P<0.05),围手术期并发症发生率明显降低(H=4.694,P<0.05)。结论 ERAS对糖尿病并结肠癌手术病人同样安全、有效,提示ERAS理念可以在糖尿病人群中推广应用。

Objective To observe the effect of Enhanced Recovery After Surgery (ERAS) on perioperative complications of colorectal cancer in diabetic patients, verify the safety and effectiveness of ERAS for diabetic patients, and further improve and spread ERAS. Methods Ninety-seven diabetic patients who underwent surgery for colorectal cancer were randomly divided into control group ( n =48) and experimental group ( n =49). ERAS was used in the treatment of the experimental group during perio- perative period, while conventional treatment was used in the control group. Results Eighty-six cases including 42 in the experimental group and 44 in the control group finished the study. Compared with the control group, the experimental group had significantly shorter time to first flatus and discharge and lower hospital costs ( t =4.218-7.621, P <0.05), as well as a significantly lower perioperative complication rate ( H =4.694, P <0.05). Conclusion ERAS is safe and effective for diabetic patients surgically treated for colorectal cancer, suggesting that ERAS can be widely applied in the diabetic population.
