摘要
目的:观察生化汤联合芬吗通预防人工流产术后宫腔粘连(intrauterine adhesion,IUA)的临床疗效。方法:将80例行人工流产术育龄期女性按照随机数字表法分为对照组与观察组,每组各40例。两者均给予常规术后芬吗通周期治疗、宫腔注射几丁糖以及抗感染治疗;观察组在此基础上联合生化汤。比较两组患者术后一般情况、首次月经恢复情况及IUA发生率,检测两组患者治疗前后血液流变学指标变化情况。结果:观察组患者术后阴道出血时间短于对照组,子宫内膜厚度及术后2周尿HCG转阴率大于对照组,差异具有统计学意义(P <0. 05)。两组患者全血低切黏度及血浆黏度低于本组治疗前,且观察组低于对照组,差异具有统计学意义(P <0. 05)。观察组患者首次月经恢复情况优于对照组,IUA发病率低于对照组,差异具有统计学意义(P <0. 05)。结论:生化汤联合芬吗通可预防人工流产术后IUA的发生,作用机制可能与改善患者的高凝状态有关。
Objective: To observe the clinical curative effect of Engendering and Transforming Decoction in combination with Fenmoston on preventing intrauterine adhesions after induced abortion. Methods: Eighty women of childbearing age who underwent induced abortion were divided into the control group and the observation group according to random number table method,with 40 cases in each group. Both groups were given Fenmoston cycle therapy,chitosan injection into uterine cavity and anti-infection treatment after routine operation. On this basis,the observation group was treated with Engendering and Transforming Decoction.Then the general condition,the first menstrual recovery and the incidence of IUA after the operation were compared between the two groups. The changes of hemorheological indexes before and after the treatment were detected. Results: The time of vaginal bleeding after the operation in the observation group was shorter than that in the control group. The thickness of endometrium and the negative rate of urinary HCG within the two weeks after the operation were higher than those in the control group. The differences were statistically significant( P < 0. 05). The whole blood low shear viscosity and plasma viscosity of the two groups were lower than those before the treatment,and the observation group was lower than the control group,the differences were statistically significant( P < 0. 05). The first menstrual recovery of the observation group was better than that of the control group,and the incidence of IUA was lower than that of the control group,The differences were statistically significant( P < 0. 05). Conclusion: Engendering and Transforming Decoction in combination with Fenmoston can prevent the occurrence of IUA after induced abortion,and its mechanism may be related to the improvement of hypercoagulability of patients.
