江浙沪地区艾滋病男男同性恋人群心理情绪与同妻关系相关性调查研究

Investigation and analysis of correlation between psychological emotion of HIV/AIDS men who have sex with men and their “wives” in Jiangsu, Zhejiang and Shanghai district

目的了解艾滋病男男同性恋同妻人群压力、焦虑、抑郁现况,探讨同妻家庭关系与以上因素的相关性。方法 2018年1～8月,方便抽样上海市公共卫生临床中心,江苏省无锡市第五人民医院,浙江省杭州市西溪医院HIV自愿咨询检测(voluntary counseling testing,VCT)门诊艾滋病男男同性恋已婚者,采用自填式问卷调查方法,调查压力、焦虑、抑郁与同妻关系的相关性。结果研究对象压力、焦虑及抑郁情绪检出率分别为68.07%、 57.58%和92.50%。艾滋病治疗时间<6个月抑郁得分(71.25±1.30)分,≥6个月抑郁得分(62.73±11.04)分,差异有统计学意义(t=9.675,P<0.01)。与妻子关系不融洽的抑郁得分(64.79±9.24)分与妻子关系融洽的抑郁得分(60.88±12.85)分,差异有统计学意义(t=2.40,P=0.014)。压力得分与"妻子知道您男男同性恋吗?"有相关性(r=-0.268);焦虑得分与"妻子知道您HIV阳性吗?"有相关性(r=-0.248);抑郁得分与妻子受教育程度有相关性(r=-0.492)。结论同妻家庭关系对患者产生压力,对治疗产生影响,需要艾滋病多学科会诊给予干预。

Objective To understand the current situation of stress, anxiety and depression of HIV/AIDS men who have sex with men(MSM), and to explore the correlation between the family relationship of their "wives" and the above factors so as to provide guidance for AIDS case management. Methods A self-reported questionnaire survey was conducted among married HIV/AIDS MSM in VCT clinics in Jiangsu, Zhejiang and Shanghai from January to August 2018 to assess their stress, anxiety, depression and to analyze the correlation of stress, anxiety, depression with their "wives". Results 68.07%, 57.58% and 92.50% of study subjects suffered from stress, anxiety and depression.The depression score was higher among HIV/AIDS MSM with 6 month or longer antiretroviral treatment compared to those with less than 6 month treatment(71.25±1.30 vs 62.73±11.04;t=9.675,P<0.01), and was also higher among HIV/AIDS MSM who had disharmonious relationship with their wives compared to those who had good relationship(64.79±9.24 vs 60.88±12.85;t=2.40, P=0.014).Stress was closely correlated with the homosexual identity uncovered by his wife(r=-0.268), while anxiety and depression were closely correlated with HIV positive identity uncovered by his wife(r=-0.248)and the education level of his wife(r=-0.492). Conclusions The family relationship of their"wives"exerts pressure on HIV/AIDS MSM and has negative impact on antiviral treatment,which requires a multi-disciplinary intervention.
