HPV16感染和雌孕激素水平与宫颈癌的相关性分析 被引量:10

Analysis on the correlation between HPV16 infection,estrogen and progesterone levels and cervical cancer

摘要 目的分析人乳头瘤病毒(HPV) 16感染和雌孕激素水平与宫颈癌的相关性,探讨感染性疾病在恶性肿瘤病理过程中的作用。方法选取2017年1月-2018年2月丽水市人民医院收治的宫颈癌患者85例作为病例组,选取同期非宫颈癌女性患者80例作为对照组。对比两组患者宫颈细胞中的HPVs DNA和HPV16 DNA阳性率及两组患者血清雌二醇(E_2)、孕酮(P)水平。结果病例组患者的HPVs DNA、HPV16 DNA阳性率72. 94%、44. 71%显著高于对照组的10. 00%、3. 75%,差异均有统计学意义(P<0. 01)。病例组患者血清E2、P水平(46. 65±25. 71) pg/ml、(2. 46±1. 35) ng/ml均显著高于对照组的(28. 03±22. 07) pg/ml、(1. 19±1. 12) ng/ml,差异均有统计学意义(P <0. 01)。Logistic多元回归分析结果显示,宫颈癌的发生与HPV16 DNA阳性、血清E2水平具有相关性(P<0. 05)。结论宫颈癌患者具有较高的HPV16感染率和血清雌孕激素水平,二者均与宫颈癌的发病具有独立相关性,可能共同参与了宫颈癌的病理过程。 Objective To analyze the correlation between human papillomavirus( HPV) 16 infection,estrogen and progesterone levels and cervical cancer,explore the role of infectious diseases in pathological process of malignant tumors. Methods From January 2017 to February 2018,85 patients with cervical cancer were selected from Lishui People’s Hospital as case group,and 80 women without cervical cancer during the same period were selected as control group. The positive rates of HPVs DNA and HPV16 DNA in cervical cells,serum estradiol( E2) and progesterone( P) levels in the two groups were detected and compared. Results The positive rates of HPVs DNA and HPV16 DNA in case group were 72. 94% and 44. 71%,respectively,which were statistically significantly higher than those in control group( 10. 00%,3. 75%)( P<0. 01). The levels of serum E2 and P in case group were( 46. 65±25. 71) pg/ml and( 2. 46±1. 35) ng/ml,respectively,which were statistically significantly higher than those in control group [( 28. 03±22. 07) pg/ml,( 1. 19±1. 12) ng/ml]( P<0. 01). Multivariate Logistic regression analysis showed that the occurrence of cervical cancer was correlated with positive HPV16 DNA and serum E2 level( P<0. 05). Conclusion The patients with cervical cancer have high HPV16 infection rate and serum estrogen and progesterone levels. The two factors are independently correlated with the occurrence of cervical cancer,which may be involved in pathological process of cervical cancer.
