股动脉局部注药治疗血栓闭塞性脉管炎的临床疗效分析

Analysis of clinical efficacy of local injection for the treatment of femoral artery vasculitis

目的探讨股动脉局部注射给药治疗血栓闭塞性脉管炎(TAO)的临床疗效。方法选取2007年2月至2013年3月甘肃省人民医院血管外科收治TAO患者86例。根据入院时病例号末尾数字进行分组,分为对照组(n=43)给予普通外周静脉滴注治疗;为研究组(n=43)给予股动脉局部注射给药。观测治疗后的静息痛、患肢冷感、跛行距离、踝肱指数(ABI)和经皮氧分压(TcPO2)治疗指标。结果治疗后,研究组各项指标有效缓解人数均高于对照组,差异有统计学意义(P<0.05);治疗后并发症中,对照组出现2例行膝关节离断截肢术,研究组3例行小腿截肢术。结论 TAO经股动脉局部注射治疗较传统外周静脉滴注治疗临床疗效好,安全有效,值得推广。

Objective To investigate the clinical efficacy of local injection of femoral artery in the treatment of thromboangiitis obliterans(TAO). Methods 86 patients with TAO admitted to the department of vascular surgery of Gansu Provincial People’s Hospital from February 2007 to March 2013 were selected. According to the number at the end of the case number at the time of admission, the patients were divided into the control group(n=43) and given the usual peripheral intravenous infusion;the study group(n=43) was given the local injection of the femoral artery. The resting pain, limb coldness, claudication distance, ankle brachial index(ABI) and transcutaneous oxygen saturation(TcPO2) were measured after treatment. Results After treatment, the effective remission rate of the study group was higher than that of the control group, the difference was statistically significant(P<0.05). In the post-treatment complications, 2 cases in the control group underwent knee amputation and 3 cases in the study group underwent leg amputation. Conclusion Local injection of TAO through the femoral artery is safer and more effective than traditional peripheral intravenous infusion. It is worthy of promotion.
