非血栓性髂静脉压迫综合征伴下肢静脉曲张腔内治疗术后疗效研究

Non-thrombotic iliac vein compression syndrome with varicose veins of the lower extremities study on the efficacy of endovascular treatment

目的探讨非血栓性髂静脉压迫综合征(NIVCS)伴下肢静脉曲张行髂静脉球囊扩张支架植入术腔内治疗联合大隐静脉高位结扎剥脱术后疗效研究。方法选取宁夏医科大学第二附属医院2018年01月至2018年10月收治的符合诊断病例数62例患者。分别于术前、术后10 d、术后3个月测量静息压,术后3个月随访,记录患者静脉曲张、水肿、静息痛、皮肤色素沉着和溃疡愈合情况。结果患者术后10 d(125.64±5.44)cm H2O和术后3月(124.86±4.31)cm H2O较术前(131.66±6.13)cmH2O下降,差异有统计学意义(P<0.05)。术后3个月随访,患肢静脉曲张改善59条,水肿完全缓解44条,疼痛完全缓解12条,色素沉着消退12条,溃疡完全愈合9条。结论髂静脉球囊扩张支架植入术腔内治疗联合大隐静脉高位结扎剥脱术治疗NIVCS伴下肢静脉曲张能有效缓解症状,值得临床推广。

Objective To investigate the efficacy of endovascular treatment of non-thrombotic iliac vein compression syndrome(NIVCS) with varicose veins of the lower extremity for iliac vein balloon stent implantation combined with high saphenous vein ligation and stripping. Methods From January 2018 to October 2018, 62 patients were selected from the Second Affiliated Hospital of Ningxia Medical University. Resting pressure was measured before operation, 10 days after operation and 3 months after operation. Varicose veins, edema, resting pain, skin pigmentation and ulcer healing were recorded after 3 months of follow-up. Results 10 days after operation(125.64±5.44)cm H2O and 3 months after operation(124.86±4.31)cm H2O were lower than those before operation(131.66±6.13)cm H2O, the difference was statistically significant(P<0.05). At 3 months follow-up, 59 varicose veins were improved, 44 edema was completely relieved, 12 pains were completely relieved, 12 pigmentation subsided and 9 ulcers healed completely. Conclusions Intraluminal treatment with iliac balloon balloon stenting combined with high saphenous vein ligation and stripping for the treatment of NIVCS with lower extremity varicose veins can effectively alleviate symptoms and is worthy of clinical promotion.
