丙种球蛋白对急性病毒性心肌炎合并完全性房室传导阻滞患儿的疗效观察

Therapeutic effect of gamma globulin on children with acute viral myocarditis complicated with complete atrioventricular block

目的探究对急性病毒性心肌炎合并完全性房室传导阻滞(CAVB)患儿实施丙种球蛋白治疗的效果。方法将2016年5月至2018年5月福建医科大学附属漳州市医院收治的70例急性病毒性心肌炎合并CAVB患儿随机分为观察组和对照组各35例,对照组应用常规治疗,观察组在对照组基础上加用丙种球蛋白,比较两组治疗效果及并发症发生率。结果治疗后,观察组患儿总有效率为94.29%,高于对照组的74.29%;观察组症状消失时间、心电图恢复正常时间、心肌酶恢复正常时间均短于对照组;观察组患儿心肌酶谱变化水平均低于对照组;观察组的外周血T细胞亚群与对照组比较,差异有统计学意义(P<0.05)。结论对急性病毒性心肌炎合并CAVB患儿实施丙种球蛋白治疗可行,可显著增强疗效,对缓解病情具有较积极的作用。

Objective To investigate the effect of gamma globulin therapy in children with acute viral myocarditis complicated with complete atrioventricular block(CAVB). Methods 70 children with acute viral myocarditis and CAVB admitted to Wuzhou Hospital of Fujian Medical University from May 2016 to May 2018 were randomly divided into observation group and control group, 35 cases in each group. The control group received routine treatment. The gamma globulin was added to the control group to compare the treatment effect and the incidence of complications between the two groups. Results After treatment, the total effective rate of the observation group was 94.29%, which was higher than that of the control group(74.29%). The disappearance time of symptoms, the normal time of electrocardiogram recovery, and the recovery time of myocardial enzyme were shorter than that of the control group. The myocardial zymogram change level was lower than that of the control group;the peripheral blood T cell subsets in the observation group were significantly different from the control group(P<0.05). Conclusion It is feasible to treat gamma globulin in children with acute viral myocarditis complicated with CAVB, which can significantly enhance the curative effect and have a positive effect on relieving the disease.
