摘要
目的探讨血清胃蛋白酶原联合胃泌素-17以及幽门螺杆菌(Hp)14碳呼气试验检测对胃癌患者的诊断效果。方法回顾性分析2015年3月至2017年2月在本院接受胃镜检查的160例胃癌高危人群的临床资料,依据病理检查结果,将入选者划分为A组(萎缩性胃炎,46例)、B组(浅表性胃炎,33例)、C组(胃溃疡,37例)、D组(胃癌,44例),而后选取同期于本院接受健康检查的20例健康者组成对照组,所有入选者均需接受血清胃蛋白酶原、胃泌素-17及幽门螺杆菌14碳呼气试验,分析检查结果。结果对照组Hp感染率均低于A组、B组、C组、D组,差异有统计学意义(P<0.05);各组胃蛋白酶原Ⅱ水平对比,差异无统计学意义;D组胃蛋白酶原Ⅰ水平均低于A组、B组、C组、对照组,差异有统计学意义(P<0.05);D组胃泌素-17水平均高于A组、B组、C组、对照组,差异有统计学意义(P<0.05)。结论血清胃蛋白酶原联合胃泌素-17及14碳呼气试验检测对胃癌早期诊断具有较高参考价值,利于临床及早采取针对性治疗。
Objective To investigate the diagnostic effect of serum pepsinogen combined with gastrin-17 and Helicobacter pylori (Hp) 14 carbon breath test in patients with gastric cancer. Methods From March 2015 to February 2017, the clinical data of 160 patients with gastric cancer who were examined by gastroscopy in our hospital from March 2015 to February 2017 were retrospectively analyzed. According to the results of pathological examination, the selected subjects were divided into group A (atrophic gastritis, 46 cases). Group B (superficial gastritis, 33 cases of), C group (gastric ulcer, 37 cases of), D group, 44 cases of gastric cancer). Serum gastrin-17, serum gastrin-17 and Helicobacter pylori 14-C breath test were used to analyze the results. Results The infection rate of Hp in control group was lower than that in A group, B group, C group and D group (P< 0.05), but there was no significant difference in pepsinogen Ⅱ level in each group. The level of pepsinogen Ⅰ in group D was lower than that in group A, B, C and control group (P<0.05). The levels of gastrin-17 in group D were significantly higher than those in group A, B, C and control group (P<0.05). Conclusion Serum stomach The detection of proproteinogen combined with gastrin-17 and 14 C breath test has high reference value in the early diagnosis of gastric cancer and is beneficial to the early treatment of gastric cancer.
