血常规和凝血检测在急性心肌梗死转归期中的临床意义

Clinical Significance of Blood Routine and Blood Coagulation Test in the Outcome of Acute Myocardial Infarction

目的探究急性心肌梗死转归期患者采取血常规与凝血检测对病情判断与预后评估的价值。方法选择2016年3月-2018年3月天津市静海区医院治疗的80例急性心肌梗死转归期患者为研究对象,按TIMI危险评分标准分为对照组和观察组,每组各40例,两组均进行血常规与凝血检测。比较两组血常规指标[血小板计数(PLT)、白细胞计数(WBC)、中性粒细胞与淋巴细胞比值(NLR)、血小板与淋巴细胞比值(PLR)]与凝血指标[抗凝血酶(ATⅢ)、纤维蛋白酶(FIB)、活化部分凝血酶活性时间(APTT)、凝血酶时间(TT)、凝血酶原活动度(PTA)]。结果观察组PLT、NLR、PLR均明显高于对照组(P<0.05);两组WBC水平比较差异无统计学意义(P>0.05);观察组ATⅢ低于对照组,FIB、APTT明显高于对照组(P<0.05);两组TT、PTA比较差异无统计学意义(P>0.05)。结论根据PLT、NLR、PLR等血常规指标与FIB、APTT、ATⅢ等凝血指标检测结果,可判断出急性心肌梗死转归期患者的危险分层及预后情况。

Objective To investigate the value of blood routine and coagulation test in the assessment of patients with acute myocardial infarction. Methods 80 patients with acute myocardial infarction who were treated in our hospital from March 2016 to March 2018 were enrolled in the study.According to TIMI risk scores,40 patients were enrolled in the study.Blood and coagulation tests were performed in both groups.The blood routine indicators,such as Platelet count (PLT),white blood cell count (WBC),neutrophil to lymphocyte ratio (NLR),platelet to lymphocyte ratio (PLR)] and coagulation index [antithrombin (ATIII)),fibrin protease (FIB),activated partial thrombin activity time (APTT),thrombin time (TT),prothrombin activity (PTA) were compared. Results The PLT,NLR and PLR of the observation group were higher than those of the control group ( P <0.05).There was no significant difference in WBC levels between the two groups ( P >0.05).The ATIII of the observation group was lower than that of the control group.The FIB and APTT were higher than that of the control group.There was no significant difference in TT and PTA between the two groups ( P >0.05). Conclusion According to the blood routine indicators such as PLT,NLR and PLR and the results of blood coagulation indexes such as FIB,APTT and ATIII,the risk stratification and prognosis of patients with acute myocardial infarction during the transition period can be judged.
