Logistic regression analysis of risk factors of poor prognosis in patients with chronic cor pulmonale

目的分析慢性肺心病患者预后不良的危险因素,并提出预防策略。方法回顾性分析2017年6月至2018年6月我院收治的88例慢性肺心病患者的临床资料,均实施心电图检查,分析影响预后不良的危险因素。结果本研究中有16例预后不良,发生率为18.18%。发生组年龄>60岁、病程>5年、伴有低氧血症、伴有肺性脑病、心电图P波异常改变的患者占比显著高于未发生组(P<0.05)。多因素Logistic回归分析显示,年龄、病程、伴有低氧血症、伴有肺性脑病以及心电图P波异常改变是影响慢性肺心病预后不良的独立危险因素(P<0.05)。结论慢性肺心病患者的预后不良发生率较高,年龄、病程、伴有低氧血症、伴有肺性脑病以及心电图P波异常改变均是其独立危险因素,可将心电图P波变化作为预后判断的参考指标,并制定有效的预防策略。

Objective To analyze the risk factors of poor prognosis in patients with chronic cor pulmonale, and put forward preventive strategies. Methods The clinical data of 88 patients with chronic cor pulmonale admitted in our hospital from June 2017 to June 2018 were retrospectively analyzed. All patients were examined by electrocardiogram, and the risk factors affecting poor prognosis were analyzed. Results In this study, there were of 16 cases poor prognosis, the incidence was 18.18%. The proportion of patients with age>60, course of disease>5 years, hypoxemia, pulmonary encephalopathy and abnormal changes of P wave in ECG in the occurrence group was significantly higher than that in the non-occurrence group(P <0.05). Multivariate Logistic regression analysis showed that age, course of disease, hypoxemia, pulmonary encephalopathy and abnormal changes of electrocardiogram P wave were independent risk factors for poor prognosis in patients with chronic cor pulmonale(P<0.05). Conclusion Patients with chronic cor pulmonale have a higher incidence of poor prognosis. Age, course of disease, hypoxemia, pulmonary encephalopathy and abnormal changes of P wave in ECG are all independent risk factors. The change of P wave in ECG can be used as a reference index for prognosis judgment, and effective prevention strategies can be formulated.
