固定义齿和可摘义齿咬合重建修复对患者满意度及咀嚼效率的影响

Effect of Occlusal Reconstruction of Fixed and Removable Dentures on Patient Satisfaction and Masticatory Efficiency

目的研究探讨固定义齿、可摘义齿咬合重建修复对咬合重建患者咀嚼效率的影响。方法方便选取2014年9月-2018年5月期间该院接收治疗的90例咬合重建患者,其中固定义齿修复组45例,可摘义齿修复组45例,对比分析修复前后两组患者的咀嚼效率。结果咬合重建修复之前,固定义齿修复组、可摘义齿修复组分别有6例、4例患者有颞下颌弹响,修复后均仅有1例还有弹响;修复之后,90例患者均感到舒适,并且咬合有力;咬合重建修复之后,固定义齿修复组患者满意度(97.78%)显著高于对照组(84.44%),差异有统计学意义(P<0.05)。与重建修复前相比,修复1个月后固定义齿修复组咀嚼效率(78.2±4.1)、可摘义齿修复组咀嚼效率(77.8±2.7),修复6个月后固定义齿修复组咀嚼效率(93.1±5.2)、可摘义齿修复组咀嚼效率(79.9±4.4)咀嚼效率均明显提高,差异有统计学意义(P<0.05)。咬合重建修复1个月后,两组患者的咀嚼效率差异无统计学意义(P>0.05);咬合重建修复6个月后,固定义齿修复组的咀嚼效率增加程度明显高于可摘义齿修复组,经统计分析,差异有统计学意义(P<0.05)。结论与可摘义齿修复咬合重建相比,固定义齿修复咬合重建可使得患者满意度与咀嚼效率更加,值得在临床上推广应用。

Objective To study the effect of occlusal reconstruction of fixed and removable dentures on masticatory efficiency of patients with occlusal reconstruction. Methods A total of 90 patients with occlusal reconstruction received in our hospital from September 2014 to May 2018 were convenient selected, including 45 patients in fixed denture restoration group and 45 patients in removable denture restoration group. The masticatory efficiency of the two groups before and after restoration was compared and analyzed. Results Before occlusal reconstruction, there were 6 cases in fixed denture restoration group and 4 cases in removable denture restoration group having temporomandibular springing. And after restoration,only 1 case had temporomandibular springing. After restoration, 90 patients felt comfortable and had strong occlusion. After occlusal reconstruction, satisfaction of fixed denture restoration group was 97.78%, which was significantly higher than of the control group(84.44%). The difference was statistically significant(P<0.05). Compared with the statistics before restoration, the mas ticatory efficiency of fixed denture restoration group after one-month restoration(78.2 ±4.1) and after six-month restoration(93.1±5.2), and the masticatory efficiency of removable denture restoration group after one-month restoration(77.8±2.7) and after six-month restoration(79.9±4.4), were increased significantly the difference was statistically significant(P<0.05). One month after occlusal reconstruction, there was no significant, difference in masticatory efficiency between the two groups(P>0.05). Six months after occlusal reconstruction, the masticatory efficiency of fixed denture restoration group was significantly higher than that of removable denture restoration group, and the difference was statistically significant(P <0.05). Conclusion Compared with removable denture, fixed denture restoration can improve satisfaction and chewing efficiency of patients. It is worthy of clinical application.
