马来酸桂哌齐特联合硫酸镁治疗蛛网膜下腔出血后脑血管痉挛的疗效

Clinical efficacy of cinepazide maleate combined with magnesium sulfate for treatment of cerebrovascular spasm of subarachnoid hemorrhage

目的研究马来酸桂哌齐特联合硫酸镁治疗动脉瘤性蛛网膜下腔出血后脑血管痉挛患者的疗效。方法方便选取2015年4月-2017年4月该医院收治的动脉瘤性蛛网膜下腔出血后脑血管痉挛患者62例,随机均分为试验组和对照组,试验组使用马来酸桂哌齐特与硫酸镁注射液进行联合治疗,对照组单独使用硫酸镁注射液治疗;以临床症状缓解情况、有效率、血流速度、不良反应等作为效能指标。结果治疗后试验组有效率为85.0%显著高于对照组60.0%,差异有统计学意义(χ^2=5.010,P<0.05);与对照组比较,试验组患者脑膜刺激明显减弱、头疼得到明显缓解、GCS评分增加,且大脑中动脉平均血流速度明显慢于对照组,差异有统计学意义(P<0.05),脑梗死与脑血管痉挛发生明显少于对照组,差异有统计学意义(P<0.05)。结论马来酸桂哌齐特结合硫酸镁治疗蛛网膜下腔出血后脑血管痉挛疗效良好,值得临床推广使用。

Objective To investigate the clinical effect of cinepazide maleate combined with magnesium sulfate injection treatment for cerebrovascular spasm of aneurysmal subarachnoid hemorrhage. Methods A total of 62 cases of cerebrovascular spasm of aneurysmal subarachnoid hemorrhage admitted to our hospital from April 2015 to April 2017 were convenient selected and randomly divided into control group(magnesium sulfate injection treatment) and experimental group(cinepazide maleate combined with magnesium sulfate injection treatment). The clinical symptoms, total effective rates,blood flow velocity and adverse events of two groups were compared. Results After treatment, the effective rate of the experimental group was 85.0%, which was significantly higher than that of the control group(60.0%). The difference was statistically significant(χ^2=5.010, P<0.05). Compared with the control group, the meningeal irritation of the experimental group was significantly weakened, the headache was significantly relieved, the GCS score increased, the mean blood flow velocity of the middle cerebral artery was significantly slower,the difference was statistically significant(P<0.05), and the incidence of cerebral infarction and cerebral vasospasm was significantly lower,the difference was statistically significant(P <0.05).Conclusion Cinepazide maleate combined with magnesium sulfate injection for the treatment of cerebrovascular spasm of aneurysmal subarachnoid hemorrhage has a significant clinical effect. It is worth of clinical promotion.
