保肝解毒汤联合西药治疗妊娠期肝内胆汁淤积症的效果分析

Effect of Baogan Jiedu Decoction Combined with Western Medicine on Patients with Intrahepatic Cholestasis During Pregnancy

目的探讨妊娠期肝内胆汁瘀积症患者在西药治疗基础上,联用中药保肝解毒汤临床效果及对预后的影响。方法便利选择妊娠期肝内胆汁瘀积症患者100例,均为该院2017年3月-2018年3月收治,随机分组,就取腺苷蛋氨酸等西药方案治疗(对照组,n=50)与取保肝解毒汤中药方案加用治疗(观察组,n=50)总有效率及预后展开对比。结果经治疗后,观察组总有效率98%高于对照组74%,差异有统计学意义(χ^2=11.96,P<0.05)。观察组肝功能指标ALT、TBA、AST、DBIL、TBIL水平,凝血因子指标PLT、FIB、PT水平改善均优于对照组(P<0.05),且不良妊娠结局率、瘙痒评分低于对照组,差异有统计学意义(P<0.05)。结论针对临床收治的妊娠期肝内胆汁瘀积症的患者,在西药治疗基础上,取中药保肝解毒汤加用,可提高总有效率,改善肝功能及凝血状况,降低剖宫产率,防范不良妊娠结局,缓解瘙痒程度,具非常重要的应用价值。

Objective To investigate the clinical effect and prognosis of traditional Chinese medicine Baogan Jiedu Decoction combined with western medicine treatment in patients with intrahepatic cholestasis during pregnancy. Methods A total of 100 patients with intrahepatic cholestasis during pregnancy admitted to our hospital from March 2017 to March 2018 were convenient selected and randomly divided. Western medicine adenosylmethionine and polyenolylcholine was used in the control group(n=50), and the Baogan Jiedu Decoction was used in the observation group(n=50). The total effective rate and prognosis were compared. Results After treatment, the total effective rate of the observation group was 98%, which was higher than that of the control group(74%), and the difference was statistically significant(χ^2=11.96, P<0.05). The liver function indexes of ALT, TBA, AST, DBIL, TBIL, and the levels of PLT, FIB and PT in the observation group were better than those of the control group(P<0.05). And the adverse pregnancy outcome rate and pruritus score in the observation group were lower than those in the control group. The difference was statistically significant(P<0.05). Conclusion For the treatment of patients with intrahepatic cholestasis during pregnancy, on the basis of Western medicine treatment, Chinese medicine Baogan Jiedu Decoction can improve the total effective rate, liver function and blood coagulation status. It can also reduce the cesarean section rate. It is of great value to prevent adverse pregnancy outcomes and relieve itching.
