黄体酮阴道凝胶联合硝苯地平片治疗先兆早产观察

Clinical Observation of Progesterone Vaginal Gel Combined with Nifedipine Tablets in the Treatment of Threatened Premature Labor

目的:观察黄体酮阴道凝胶联合硝苯地平片治疗先兆早产患者的临床疗效。方法:120例先兆早产患者随机分成对照组和观察组,每组各60例。对照组采用硝苯地平片治疗,起始剂量为20 mg po,然后调整为10 mg,po,tid,持续48 h;观察组在对照组基础上每天加用黄体酮阴道缓释凝胶1支塞阴道,持续用至孕35周。比较两组延长妊娠时间、≥35周分娩率、新生儿体质量以及新生儿窒息率和药品不良反应等指标。结果:观察组患者延长妊娠时间、≥35周分娩率、新生儿体质量等指标均明显优于对照组(P<0.01)。两组新生儿窒息率、药品不良反应发生率比较,差异无统计学意义(P>0.05)。结论:对于先兆早产患者,通过黄体酮阴道凝胶联合硝苯地平片治疗能够有效延长患者的孕周,增加新生儿的体质量,改善妊娠结局,值得推广。

Objective:To investigate the clinical efficacy of progesterone vaginal gel combined with nifedipine tablets in the treatment of patients with threatened preterm labor.Methods:A total of 120 patients with threatened preterm labor were randomly divided into the control group and the study group with 60 cases in each.The control group was treated with nifedipine tablets with the initial dose of 20 mg,and then 10 mg per time,3 times one day for 48 hours.The study group was treated with progesterone sustained-release vaginal gel combined with nifedipine tablets,once a day vaginally,till the 35 thgestational week.The extended pregnancy time,≥35 W delivery rate,neonatal weight,neonatal asphyxia rate and adverse drug reactions were compared between the groups.Results:In the study group,the prolonged pregnancy time,≥35 W delivery rate and neonatal weight were significantly higher than those of the control group(P<0.01).There was no significant difference in the neonatal asphyxia rate between the groups(P>0.05).There were no significant adverse drug reactions in the two groups.Conclusion:For patients with threatened preterm labor,the treatment with progesterone vaginal gel combined with nifedipine tablets can effectively prolong the gestational age and increase the weight of newborns.The clinical effect is significant,which is worthy of promotion.
