四物汤联合乳果糖治疗妊娠期便秘(血虚阴亏)随机平行对照研究

Siwu Decoction(四物汤) combined with Lactulose in the Treatment of Pregnancy Constipation(Blood Deficiency and Yin Deficiency/血虚阴亏) Randomized Controlled Study

[目的]观察四物汤联合乳果糖治疗妊娠期便秘(血虚阴亏)疗效。[方法]使用随机平行对照方法,将60例门诊患者按就诊顺序号抽签简单随机分两组,饮食清淡,忌寒凉,调畅情志,合理运动,规律作息,发现任何身体不适,立刻停止用药。对照组30例乳果糖,10mL/次,3次/d。治疗组30例四物汤(当归9g,川芎6g,白芍9g,地黄~熟15g),血虚加鹿角胶、阿胶等,或适当加人参、黄芪等补气之品;冲任虚损、胎漏下血则加阿胶、艾叶、甘草;气血两虚加四君子汤,即八珍汤;1剂/d,水煎300mL,3次/d;乳果糖治疗同对照组。连续治疗1周为1疗程。观测临床表现、中医症候积分、不良反应。连续治疗3疗程,判定疗效。[结果]治疗组临床痊愈5例,显效10例,有效12例,无效1例,总有效率96.43%;对照组临床痊愈1例,显效9例,有效13例,无效6例,总有效率79.31%;治疗组疗效优于对照组(P<0.05)。中医症候积分两组均有降低(P<0.01),治疗组降低低于对照组(P<0.01)。[结论]四物汤联合乳果糖治疗妊娠期便秘(血虚阴亏),疗效满意,无严重不良反应,值得推广。

[Objective] To observe the curative effect of Siwu decoction combined with lactulose in the treatment of pregnancy constipation(blood deficiency and Yin deficiency).[Methods] Using the random parallel control method, 60 outpatients were randomly divided into two groups by drawing lots according to the order of visiting number. The patients had a light diet, avoided cold and cool, adjusted their mood,exercised reasonably and kept regular hours. If they found any physical discomfort, they would immediately stop medication. Lactulose in the control group was 10 mL/time and 3 times/day. In the treatment group,30 cases were treated with siwu decoction(9 g angelica, 6 g chuanxiong, 9 g paeony alba and 15 g rehmannia glutinosa). Chong ren virtual loss, fetal leakage under the blood then add Ajiao, artemisia argyi, licorice;Qi and blood two empty add four gentleman soup, namely eight precious soup;decoction: 1 dose/d, 300 mL water,3 times/d;Lactulose was treated the same as the control group. Continuous treatment for 1 week for a course of treatment. Clinical manifestations, TCM syndrome scores and adverse reactions were observed. Continuous treatment for 3 courses, to determine the efficacy.[Results] In the treatment group, 5 cases were cured, 10 cases were effective, 12 cases were effective, 1 case was ineffective, and the total effective rate was 96.43%. In the control group, 1 case was cured, 9 cases were effective, 13 cases were effective, 6 cases were ineffective,and the total effective rate was 79.31%. The efficacy of treatment group was better than that of control group(P<0.05). The TCM syndrome score was lower in both groups(P<0.01), and lower in the treatment group than in the control group(P<0.01).[Conclusion] The curative effect of Siwu decoction combined with lactulose in the treatment of pregnancy constipation(blood deficiency and Yin deficiency) is satisfactory without serious adverse reactions, which is worthy of promotion.
