针灸结合手法整复治疗成人寰枢关节半脱位的临床研究

Clinical Study on Treatment of Adult Atlantoaxial Subluxation by Acupuncture and Moxibustion Combined with Manipulation

目的 探究针灸结合手法整复治疗成人寰枢关节半脱位的临床研究。方法 选取本院2016年8月—2017年9月寰枢关节半脱位患者60例,随机分为对照组30例和治疗组30例。对照组患者采用手法整复治疗寰枢关节半脱位,治疗组患者在手法整复的基础上采用针刺和热敏灸治疗寰枢关节半脱位,对比2组患者临床总有效率、复发率、寰枢椎张口位X线片的检查结果、颈部两侧肌肉的表面肌电振幅和频率斜率。结果 治疗后,治疗组患者临床总有效率为96.7%,对照组患者临床总有效率为83.3%,治疗组高于对照组(P<0.05);治疗组患者3个月内复发率为3.3%,对照组患者3个月内复发率为20%,治疗组低于对照组(P<0.05);治疗组患者寰枢椎张口位X线片检查结果明显低于对照组患者(P<0.05);治疗组患者颈部两侧肌肉的表面肌电振幅和频率斜率明显高于对照组患者(P<0.05)。结论 针灸结合手法整复治疗成人寰枢关节半脱位临床疗效良好、复发率低,值得临床推广使用。

Objective: To study the clinical effect of acupuncture and noxibustion combined with manipulation on the treatment of adult atlantoaxial subluxation patients. Methods: 60 patients with atlantoaxial subluxation from August 2016 to September 2017 were randomly divided into a control group and a treatment group, 30 cases per group. The patients in the control group were given manipulation for the treatment of atlantoaxial subluxation and the patients in the treatment group were given acupuncture and heat-sensitive moxibustion for the treatment of atlantoaxial subluxation. The total effective rate and recurrence rate, X-ray examination results of the atlantoaxial position and the surface myoelectric amplitude and frequency slope of the muscles on both sides of the neck of the two groups were compared. Results: After treatment, the total effective rate was 96.7% in the treatment group and 83.3% in the control group. The treatment group was higher than the control group ( P <0.05). The treatment group had a recurrence rate of 3.3% within 3 months and the recurrence rate of the control group was 20% within 3 months, and the treatment group was lower than the control group ( P <0.05). The X-ray examination results of atlantoaxial position in the treatment group were significantly lower than those in the control group ( P <0.05). The surface myoelectric amplitude and frequency slope of the muscles on both sides of the neck in the treatment group were significantly higher than those in the control group ( P <0.05). Conclusion: Acupuncture and noxibustion combined with manipulation to treat adult atlantoaxial subluxation has good clinical effect and low recurrence rate, which is worthy of clinical application.
