高频超声检查在乳腺良、恶性结节鉴别诊断中的应用效果

Application effects of high-frequency ultrasonography in differential diagnosis of benign and malignant breast nodules

目的:观察高频超声检查在乳腺良、恶性结节鉴别诊断中的应用。方法:回顾性分析2017年1月至2018年7月收治的80例(114个)乳腺结节患者的临床资料。对所有患者均进行高频超声检查,以病理学检查结果为金标准。观察高频超声检查在良恶性结节鉴别诊断中的效果;比较良恶性结节的超声影像学特征和血流参数(血流阻力指数、血流峰值流速)。结果:高频超声检查在诊断乳腺良恶性结节的敏感度为96.0%(24/25),特异度为92.1%(82/89),准确度为93.0%(106/114)。恶性结节形态规则、边界清晰比率明显低于良性结节,微钙化、后方回声衰减、血流检出率、血流阻力指数、血流峰值流速均明显高于良性结节,差异有统计学意义(P<0.05)。结论:高频超声检查对乳腺良、恶性结节具有良好的鉴别诊断效果。

Objective: To explore application of high-frequency ultrasonography in differential diagnosis of benign and malignant breast nodules. Methods: The clinical data of 80 patients (114 nodules) with breast nodules admitted from January 2017 to July 2018 were retrospectively analyzed. High-frequency ultrasonography was performed on all patients, and the pathological findings were used as the gold standard. The effects of high-frequency ultrasonography in the differential diagnosis of the benign and malignant nodules were compared. Further, the ultrasound imaging features and blood flow parameters (blood flow resistance index, peak flow velocity) of the benign and malignant nodules were compared. Results: The sensitivity, specificity and accuracy of high-frequency ultrasonography in the diagnosis of the benign and malignant breast nodules were 96.0%(24/25), 2.1%(82/89) and 93.0%(106/114), separately. The ratios of the malignant nodules with regular morphology and clean boundary were significantly lower than those of the benign nodules;the ratios of microcalcification and posterior echo attenuation, blood flow detection rate, blood flow resistance index, and peak flow velocity of the malignant nodules with were significantly higher than those of the benign nodules;and the differences were statistically significant (P<0.05). Conclusions: High-frequency ultrasonography has a good differential diagnosis effect on the benign and malignant breast nodules.
