血小板输注对ITP的免疫调节作用研究

Clinical Research of Platelet Transfusion on Immunomodulatory Effect in Treatment for Idiopathic Thrombocytopenic Purpura

目的探讨血小板输注对特发性血小板减少性紫癜(ITP)患者免疫功能的影响。方法选取本院2015年9月-2018年9月特发性血小板减少性紫癜患者78例,采用随机数字表法分为两组,对照组39例实施常规疗法,观察组39例联用血小板输注,于治疗前后行免疫功能检测,比较两组治疗效果。结果两组治疗后CD3^+、CD4^+、CD4^+/CD8^+较治疗前增加(P<0.05)。两组治疗后CD8^+较治疗前降低(P<0.05)。观察组治疗后CD3^+、CD4^+、CD4^+/CD8^+高于对照组(P<0.05)。观察组治疗后CD8^+低于对照组(P<0.05)。观察组血小板数量达正常时间、血小板数量达峰值时间早于对照组(P<0.05)。观察组血小板峰值、血小板矫正增加指数1h≥7.5×10^9/L患者所占比例高于对照组(P<0.05)。结论血小板输注可有效调节特发性血小板减少性紫癜患者的免疫功能,缩短治疗时间。

Objective To investigate influence of platelet transfusion on immune function in treatment for patients with idiopathic thrombocytopenic purpura.Methods 78 patients with idiopathic thrombocytopenic purpura were selected in hospital from September 2015 to September 2018,who were divided into two groups by random number table method.39 patients received conventional treatment as control group.39 patients combined with platelet transfusion as observation group.Immune function was tested before and after treatment.Treatment effect were compared between two groups.Results CD3^+,CD4^+,CD4^+/CD8^+ in two groups increased significantly after treatment (P < 0.05).CD8^+ in two groups decreased significantly after treatment (P < 0.05).CD3^+,CD4^+,CD4^+/CD8^+ in observation group were higher than control group after treatment (P < 0.05). CD8^+ in observation group was lower than control group after treatment (P < 0.05).Platelet count reached normal time,platelet count reached peak time in observation group were earlier than control group (P < 0.05). Platelet peak value,percentage of patients with platelet correction increase index 1 h (≥ 7.5×10^9/L) in observation group were higher than control group (P < 0.05).Conclusion Platelet transfusion can effectively regulate immune function of patients with idiopathic thrombocytopenic purpura and shorten treatment time.
