ANGPTL 3和ECE-1在食管癌患者中的表达及临床意义 被引量:1

摘要 目的检测血管生成素样蛋白3(ANGPTL 3)、内皮素转化酶-1(ECE-1)在食管癌(EC)患者癌组织中的表达情况,并探究其临床意义。方法选取2011年1月-2013年1月保定市第二中心医院95例EC患者的癌组织及其癌旁正常组织的石蜡标本,采用免疫组织化学法检测石蜡标本中ANGPTL 3、ECE-1表达情况,分析ANGPTL 3、ECE-1表达与EC患者临床病理特征的关系,采用Pearson相关分析方法分析EC癌组织中ANGPTL 3、ECE-1表达的相关性,采用Kaplan Meier法及Cox回归模型分析ANGPTL 3、ECE-1表达对EC患者预后的影响。结果 EC癌组织中,ANGPTL 3、ECE-1的阳性表达率均高于癌旁正常组织(P <0.05)。EC癌组织中,ANGPTL 3表达与肿瘤大小有关(P <0.05),ECE-1表达与肿瘤远处转移有关(P <0.05),且ANGPTL 3与ECE-1表达呈正相关(r=0.610,P=0.000)。Log-rank检验结果显示,ANGPTL 3阳性表达患者的术后总生存率和平均生存时间均低于ANGPTL 3阴性表达患者(P <0.05),且ECE-1阳性表达患者的术后总生存率和平均生存时间均低于ECE-1阴性表达患者(P <0.05)。Cox回归分析显示:远处转移、ANGPTL 3阳性表达、ECE-1阳性表达是EC患者生存预后的危险因素(P <0.05)。结论 EC癌组织中ANGPTL 3、ECE-1均呈高表达状态,两者呈正相关,且ANGPTL3、ECE-1高表达均与患者不良预后相关。ANGPTL 3、ECE-1具有作为EC患者病情评估、预后判断及治疗靶点的潜能。 Objective To investigate the significance of angiopoietin like protein 3 (ANGPTL 3) and endothelin converting enzyme-1 (ECE-1) in patients with esophageal carcinoma (EC). Methods A total of 95 patients with EC who were admitted in our hospital for surgery from January 2011 to January 2013 were involved in this study. Carcinoma tissues and the adjacent normal tissues were collected for identification of ANGPTL3 and ECE-1 protein expressions by immunohistochemistry assay. Correlation of ANGPTL 3/ECE-1 with the clinical characteristics was analyzed. Kaplan Meier survival model and Cox regression analysis were used to analyze the effect of ANGPTL 3 and ECE-1 expression on the prognosis of EC patients. Results Positive expression rates of ANGPTL 3 and ECE-1 in the EC carcinoma tissues were significantly higher than those in the adjacent normal tissues (P < 0.05). ANGPTL 3 expression was positively correlated with the tumor size and distant metastases. Moreover, there was a positive correlation between the expression of ANGPTL 3 and ECE-1 (r = 0.610, P = 0.000). The Log-rank results showed that the total survival rate and average survival time in patients with ANGPTL3 or ECE-1 positive expression were significantly lowered than those in patients with ANGPTL 3 negative expression (P < 0.05). Cox regression showed that distant metastasis, ANGPTL 3 and ECE-1 positive expression were risk factors for survival and prognosis of patients with EC (P < 0.05). Conclusions Increased expression of ANGPTL 3 and ECE-1 is associated with poor prognosis, which can potentially be used as diagnostic biomarkers.
