妊娠期糖尿病患者内脂素、脂联素、AFABP的变化及与胰岛素抵抗的关系

Changes of Visfatin,Adiponectin and AFABP in Gestational Diabetes Mellitus and their Relationship with Insulin Resistance

目的 探讨妊娠期糖尿病(GDM)患者血清内脂素、脂联素、脂肪细胞型脂肪酸结合蛋白(AFABP)的变化及与患者胰岛素抵抗的关系。方法 选取我院(2016.01~2017.01)确诊的GDM患者100例(GDM组)、同期妊娠的健康妇女100例(对照组),检测两组的血清内脂素、脂联素、AFABP、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FPG)、空腹胰岛素(Fins)、胰岛素抵抗指数(HOMA-IR)水平,并分析血清内脂素、脂联素、AFABP与HOMA-IR的关系。结果GDM组的血清TG、TC、HDL-C、LDL-C、FPG、Fins、HOMA-IR水平与对照组比较,差异无统计学意义(P>0.05);GDM组的血清TG、LDL-C、FPG、Fins、HOMA-IR、内脂素、AFABP水平均高于对照组,血清脂联素水平低于对照组,差异有统计学意义(P<0.05);GDM组的血清内脂素、AFABP水平与HOMA-IR呈显著的正相关关系(P<0.05),GDM组的血清脂联素水平与HOMA-IR呈显著的负相关关系(P<0.05)。结论 GDM患者血清内脂素、AFABP水平升高,脂联素水平降低,并且与GDM患者发生胰岛素抵抗具有一定的相关性。

Objective To investigate the changes of serum visfatin,adiponectin and adipocyte fatty acid binding protein(AFABP)and their relationship with insulin resistance(IR)in patients with gestational diabetes mellitus(GDM). Methods A total of 100 GDM patients(GDM group)diagnosed in our hospital(2016.01-2017.01)and 100 healthy women(control group)who were pregnant at the same time were selected.The serumvisfatin,adiponectin,AFABP,triglyceride(TG),total cholesterol(TC),high density Lipoprotein cholesterol(HDL-C),low density Lipoprotein cholesterol(LDL-C),fasting blood glucose(FBG),fasting insulin(Fins),insulin resistance index(HOMA-IR)were measured in the two groups and the relationships between serum visfatin,adiponectin,AFABP and HOMA-IR were analyzed. Results There was no significant difference in the level of serum TG,TC,HDL-C,LDL-C,FPG,Fins,HOMA-IR between the GDM group and the control group( P >0.05).The levels of TG,LDL-C,FPG,Fins,HOMA-IR,visfatin,and AFABP in GDM group were higher than those in control group,and the serum adiponectin level was lower than that in control group( P <0.05).There was a significant positive correlation between the levels of serumvisfatin,AFABP and HOMA-IR in GDM group( P <0.05).There was a significant negative correlation between adiponectin level and HOMA-IR. Conclusion The levels of serum visfatin and AFABP were increased and the level of adiponectin was decreased,and there was a certain correlation between them and insulin resistance in GDM patients.
