宫颈癌根治术中两点法腰硬联合麻醉与全麻对患者镇痛效果的影响

Effects of two-point combined spinal-epidural anesthesia and general anesthesia on stress response and analgesic effect in patients undergoing radical operation for cervical cancer

目的:探究宫颈癌根治术中两点法腰硬联合麻醉与全麻对患者应激反应及镇痛效果的影响。方法:选取宫颈癌患者147例随机分为两点法组和全麻组,两点法组在宫颈癌根治术中采用两点法腰硬联合麻醉,全麻组采用气管插管全麻方法,比较两组患者在手术过程中的平均动脉压(average arterial pressure,MAP)、心率(heart rate,HR)、血氧饱和度(blood oxygen saturation,SpO2)情况以及术后的镇痛效果。结果:两点法组患者麻醉后MAP、HR和SpO2水平与麻醉前相比差异无统计学意义( P >0.05),全麻组患者麻醉后与麻醉前MAP、HR和SpO2水平相比差异无统计学意义( P > 0.05 );两组患者麻醉后MAP、HR和SpO2水平差异有统计学意义( P <0.05);两点法组患者术后镇痛效果优于全麻组( P <0.05)。结论:在宫颈癌根治术中采用两点法腰硬联合麻醉可更有效地减轻患者的应激反应,而且术后镇痛效果更好,临床上值得应用。

Objective:To explore the effect of two-point combined spinal-epidural anesthesia and general anesthesia on stress response and analgesic effect in patients undergoing radical operation for cervical cancer. Methods:147 patients with cervical cancer were randomly divided into two-point method group and general anesthesia group. The two-point method group received two-point combined spinal-epidural anesthesia during radical operation of cervical cancer, and the general anesthesia group received general anesthesia through tracheal intubation. The average arterial pressure (MAP), heart rate (HR), blood oxygen saturation (SpO 2) and the analgesic effect after operation were compared between the two groups. Results:After anesthesia, the levels of MAP, HR and SpO2in the two-point group were slightly lower than those before anesthesia ( P >0.05), while those in the general anesthesia group were slightly higher than those before anesthesia ( P >0.05). The analgesic effect of the two-point method group was significantly better than that of the general anesthesia group ( P <0.05). Conclusion: Compared with general anesthesia, two-point combined spinal-epidural anesthesia in radical operation of cervical cancer can effectively alleviate the stress reaction of patients, and has a better analgesic effect after operation, which is worthy of further application in clinic.
