小儿重症腺病毒肺炎混合感染特征、影响因素

Characteristics, influencing factors and management of mixed infection with severe adenovirus pneumonia in children

目的:调查分析小儿重症腺病毒肺炎混合感染特征、影响因素。方法:将103例小儿重症腺病毒肺炎作为观察组,选取同期收治的103例小儿非重症腺病毒肺炎作为对照组,分析两组混合感染特征、影响因素及管理。结果:观察组患儿的年龄>3个月、热程>6d、先天性心脏病、营养不良、免疫功能低下、先天性气道发育异常、手术史、反复肺部感染发生率高于对照组(P<0.05)。结论:革兰氏阴性菌、真菌是小儿重症腺病毒肺炎混合感染特征,热程>6d、先天性气道发育异常、先天性心脏疾病、营养不良、反复肺部感染是小儿重症腺病毒肺炎的混合感染的高危因素。

Objective: To investigate the characteristics, influencing factors and management of mixed infection with severe adenovirus pneumonia in children. Methods: By retrospective analysis,103 children with severe adenovirus pneumonia admitted were selected as observation group, and103 children with non- severe adenovirus pneumonia admitted in the same period were selected as control group. The characteristics, influencing factors and management of mixed infection in the two groups were analyzed. Results: The incidence of age > 3 months, fever duration > 6 days, congenital heart disease, malnutrition, low immune function, congenital airway dysplasia, surgical history and recurrent pulmonary infection in the observation group were significantly higher than those in the control group ( P <0.05). Conclusion: Gram-negative bacteria and fungi are the characteristics of mixed infection in children with severe adenovirus pneumonia. The high risk factors of mixed infection in children with severe adenovirus pneumonia are fever duration > 6 days, congenital airway dysplasia, congenital heart disease, malnutrition and repeated pulmonary infection.
