基于三维质量评价模式的延续照护在溃疡性结肠炎患者中的应用

Application of continuous care based on three-dimensional quality evaluation model in patients with ulcerative colitis

目的分析基于三维质量评价模式的院外延续照护在溃疡性结肠炎患者中的应用效果。方法选择2015年10月—2017年9月南京医科大学附属无锡人民医院收治的溃疡性结肠炎患者120例,将2015年10月—2016年9月的患者设为对照组(n=59),将2016年10月—2017年9月的患者设为观察组(n=61),观察组患者随访过程中有2例脱落,最终纳入分析的观察组患者59例。对照组患者接受常规的住院期间症状管理及出院后每月定期的电话随访,观察组在对照组基础上实施基于三维质量评价模式的院外延续照护。在干预前和干预后第3个月末,采用炎症性肠病用生存质量表、自我护理能力量表、焦虑自评量表和抑郁自评量表对两组患者进行测评并比较。结果观察组干预后的生存质量及自我护理能力各维度评分均高于对照组(P<0.05),焦虑自评量表、抑郁自评量表评分低于对照组(P<0.05)。结论基于三维质量评价模式的院外延续照护应用于溃疡性结肠炎患者的院外照护中,能提高患者的自我护理能力,且能改善其生活质量和负性情绪,值得临床推广应用。

Objective To analyse the effect of continuous care based on three-dimensional quality evaluation model of patients with ulcerative colitis.Methods A total of 120 patients with ulcerative colitis admitted in Wuxi People’s Hospital from October 2015 to September 2017 were enrolled in this study.59 cases from October 2015 to September 2016 were chosen as control group.61 cases from October 2016 to September 2017 were chosen as observation group,which ultimately had 59 cases because of 2 cases separated.Traditional nursing care during hospitalization was carried out on the control group and a telephone interview was given to them once a month after discharge.The observation group was implemented an out-of-hospital continuous care based on the three-dimensional quality evaluation model besides the traditional nursing intervention of the control group.Before intervention and at the end of the third month after intervention,all cases of both groups were evaluated by the quality of life scale for inflammatory bowel disease,self-care ability scale,Self-rating Anxiety Scale(SAS),Self-rating Depression Scale (SDS).Results The scores of quality of life and self-care ability of observation group after intervention were higher than those of the control group (P<0.05).SAS and SDS scores of observation group were lower than those of the control group (P<0.05).Conclusion Continuous care based on the three-dimensional quality evaluation model on patients with ulcerative colitis can improve their self-care ability,quality of life and their negative emotion,which is worthy of clinical application.
