双耳同步声刺激记录的听性脑干反应在听力残疾分级评定中的应用

The Preliminary Application of Auditory Brainstem Responses Recorded from Binaural Simultaneous Stimuli in Hearing Disability Grading

摘要 目的探讨双耳同步声刺激下双通道记录的双侧听性脑干反应(bilateral auditorv brainstem response, bABR)在中国听力残疾分级评定中的应用价值。方法受试者为2017年2月-2017年12月来苏北人民医院耳鼻咽喉头颈外科行听性脑干反应(auditory brainstem response,AB R)检查的27例患者,随机分成组1 (14例)和组2 (13例)。所有受试者口服10%水合氯醛溶液10 ml,然后行交叉的iABR、bABR检查。记录被试在iABR及bABR中获得的两耳V波反应阂值及此反应阈值处的V波幅度,并对结果进行归纳和统计分析。结果所有受试者bABR中两侧的V波反应阂值相等且与iABR中好耳侧的V波反应阈值一致。两组表现出的趋势一致,当iABR中两耳V波反应阈值相等时,bABR中两侧V波反应阈值处的V波幅度显著高于iABR中两耳V波反应阈值处的V波幅度(P<0.01);当iABR中两耳V波反应阈值不等时,bABR中两侧V波反应阈值处的平均V波幅度与iABR中好耳侧V波反应阈值处的V波幅度之间无统计学差异(P>0.05)。结论bABR中V波反应阈值处的V波幅度具有稳定优势,bABR、iABR均可在听力残疾分级评定中发挥重要的作用。 Objective To examine the applied value of bilateral auditory brainstem response(bABR) in China’s hearing disability grading Scheme. Methods Subjects were 27 patients from the department of Otolaryngology, Head and Neck Surgery at Subei People’s Hospital during February to December 2017, and they were randomly divided into Group One(n=14) and Group Two(n=13). All Subjects were orally administered 10 mL of 10 % Chloral Hydrate solution and then were assessed using crossover iABR and bABR. The subjects in Group One were tested using iABR, following immediately by bABR. The Subjects in Group Two were tested in the reversal order. The Wave V thresholds obtained from iABR and bABR as well as Wave V magnitudes were recorded. The results were summarized and analyzed with professional statistical software. Results For all subjects, the Wave V thresholds from both sides in bABR were equal to the Wave V thresholds from the side ipsilateral to the good ear in iABR. Group One and Group Two showed the same trend as follows: when the Wave V thresholds from both sides in iABR were equal, the Wave V amplitudes at Wave V threshold levels from both sides in bABR were significantly larger than those in iABR(P<0.01),The Wave V thresholds from both sides in iABR were unequal, there were no statistically significant difference between the averaged Wave V amplitudes at Wave V threshold levels from the both sides in bABR and the Wave V amplitudes at Wave V threshold levels from the side ipsilateral to the good ear in iABR(P>0.05). Conclusion In bABR the Wave V magnitudes at Wave V threshold levels hold reliable advantages as stated above.Both bABR and iABR can play an important role on hearing disability grading.
