早期离床活动对股骨转子间骨折患者术后康复效果的影响

Influence of early out-of-bed activities on postoperative rehabilitation effect of patients with intertrochanteric fractures

目的 探讨早期离床活动对股骨转子间骨折患者术后康复效果的影响。方法 选取2017年3月至2018年3月于我院接受治疗的股骨转子间骨折患者146例为研究对象,随机将其等分为对照组和试验组,对照组实施常规护理,术后1周进行被动式行走训练,试验组在对照组基础上实施早期离床活动训练。比较两组患者髋关节功能水平、巴氏指数(Barthel)评分、并发症。结果 术后1个月,两组髋关节功能中的疼痛、畸形比较差异无统计学意义(P>0.05),试验组患者的功能性活动、活动度评分及总分均高于对照组(P<0.05);术后3,6个月试验组患者Barthel评分均高于对照组(P<0.05);试验组患者并发症发生率低于对照组(P<0.05)。结论 早期离床活动能够提高股骨转子间骨折患者髋关节功能水平,有效降低术后并发症发生率,具有一定的临床推广价值。

Objective To explore the influence of early out-of-bed activities on postoperative rehabilitation effect of patients with intertrochanteric fractures. Methods To select 146 patients with femoral intertrochanteric fractures who were treated in our hospital from March 2017 to March 2018 as the study subjects. They were randomly divided into the control group and the experimental group with the same number of patients. The control group was given routine nursing, and at one week after the surgery, patients in the group conducted passive waling training. On the basis of this, the experimental group was given early out-of-bed activities training. The hip function level, Barthel index score, and complications of patients in the two groups were compared. Results One month after operation, the comparative difference of pain and deformity in the hip joint function between the two groups of patients had no statistical significance( P >0.05). The functional activity, activity score and total score of the experimental group were higher than those of the control group( P <0.05). At three months and six months after the operation, the Barthel score in the test group was higher than that in the control group( P <0.05). The incidence of complications in the experimental group was lower than that in the control group( P <0.05). Conclusion Early out-of-bed activities can improve the hip joint function level in patients with intertrochanteric fractures. What's more, it can reduce the incidence of postoperative complications effectively. Therefore, such kind of treatment has certain clinical promotion value.
