IL-6与卒中后癫痫认知功能障碍的相关性研究

Elevated IL-6 predicts cognitive impairment in patients with post-stroke epilepsy

目的研究卒中急性期患者白介素6(IL-6)水平与卒中后癫痫认知功能障碍的相关性。方法纳入湖北省中西医结合医院2015-01—2016-12诊治的既往无癫痫史、确诊为卒中后癫痫发作的患者65例为研究对象,记录患者者卒中发作7 d内的IL-6水平。记录、随访患者卒中后2 a内的认知水平,经条目筛选表(six-item screener,SIS)评估患者是否存在认知功能障碍,以Logistic多因素回归分析IL-6与卒中后癫痫认知功能障碍的相关性。结果纳入的65例卒中后癫痫患者中21例2 a随访期间出现认知功能障碍,伴认知功能障碍的患者入院时IL-6水平显著高于认知功能正常的患者,且以多因素回归分析校正相关混杂因素后这一趋势仍存在。结论卒中急性期的IL-6水平与卒中后癫痫伴随的认知功能障碍存在相关性,为探索此类患者认知损害的炎性预测因子提供了依据。

Objective To analyze the correlation between blood interleukin- 6 concentration in the acute period of stroke and cognitive decline in patients with post stroke epilepsy. Methods 65 patients without previous history of epilepsy who were diagnosed with post stroke epilepsy from January 2015 to December 2016 in our hospital were included in this study,and the blood IL- 6 concentration of the subjects within 7 days after the stroke was recorded.The cognitive level of the patients at 2 years after stroke was recorded and followed up.The presence of cognitive decline was assessed by the six item screener (SIS),and the correlation between IL- 6 and cognitive decline after stroke was analyzed by Logistic multivariate regression. Results Among the 65 patients with poststroke epilepsy,21 showed cognitive dysfunction during 2 year follow up.Patients with cognitive decline showed significantly higher IL -6 level on admission than patients with normal cognitive function,and this trend still existed after multivariate analysis and adjustment of related confounders. Conclusion The study found that IL -6 level in the acute phase of a stroke may be correlated with the cognitive dysfunction associated with post stroke epilepsy,providing a basis for exploring the inflammatory predictors of cognitive impairment in such patients.
