缺血性脑卒中伴代谢综合征临床分析

Analysis of clinical feature,treatment and prognosis in patients with ischemic stroke and MS

目的 探讨缺血性脑卒中伴代谢综合征(MS)的临床特点,治疗以及预后临床疗效。方法 以郑州市中心医院收治的244例缺血性脑卒患者为研究对象,根据代谢综合征诊断标准,将患有代谢综合征的患者100例分为代谢综合征组(MS组),其他无代谢综合征患者144例分为非代谢综合征组(非MS组),2组患者接受相同治疗、护理和健康教育。比较2组治疗前的临床特点、治疗前后神经功能缺损评分(NHSS)和疗效评定。结果 临床资料显示,2组患者在发病至入院时间、身高、体质量、舒张压、空腹血糖、高密度脂蛋白胆固醇、脑卒中家族史、高血压史、房颤方面无显著性差异(P<0.05),而MS组患者在腰围、收缩压、甘油三酯含量、高血糖和糖尿病史方面与非MS组存在显著差异(P<0.05);治疗前2组NHSS评分无显著性差异(P>0.05),治疗后,2组NHSS评分显著降低(P<0.05);且治疗后非MS组降低水平显著低于MS组(P<0.05);治疗后,MS治愈14例,显效26例,有效34例,无效26例,总有效率74%,而非MS组分别为29例,64例,38例,13例,90.97%,显著高于MS组(P<0.05)。结论 缺血性脑卒中伴代谢综合征患者危险系数高,临床治疗效果差。

Objective To investigate the clinic feature,treatment and prognosis in patients with ischemic stroke and MS. Methods 244 cases with ischemic stroke and MS in our hospital were selected and divided into MS group (100 cases) and non MS group (114 cases) according to diagnostic criteria of MS.They were all given the same treatment,nursing and health education.Clinic feature,NHSS and treatment evaluation of the two groups were compared. Results Clinic data showed that,time from attack to admission,height,weight,diastolic blood pressure,FBG,HDL,family history of stroke,hypertension history,atrial fibrillation of the two groups had no statistic significance ( P <0.05),difference of waistline,systolic pressure,HTGC,hyperglycemia,diabetes history of patients between the two groups had statistic significance ( P <0.05);before treatment,NHSS score of the two groups had no statistic significance ( P >0.05),after treatment,NHSS of the two groups were obviously declined ( P <0.05);reduction level of non MS group was significantly lower than MS group after treatment ( P <0.05);in MS group,14 cured,26 excellent,34 valid,26 invalid,total efficiency 74%,in non MS group,there were 29 cases,64 cases,38 cases,13 cases,90.97% respectively,and significantly higher than MS group ( P <0.05). Conclusion Patients with ischemic stroke and MS have higher risk coefficient and worse clinic treatment effect.
