健脾消饮方联合恩度治疗恶性胸腹水的疗效观察

Effect of Jianpi Xiaoyin decoction combined with Endostar perfusion on malignant pleural effusion and ascites

目的:观察健脾消饮方联合恩度治疗恶性胸、腹水的临床疗效。方法:将68例恶性胸、腹水患者随机分为中药联合组和单灌组,两组各34例,单灌组采用恩度灌注,中药联合组在对照组的基础上服用健脾消饮方。治疗后评价两组患者的胸腹水临床疗效、生活质量、中医证候疗效、主要中医证候积分、不良反应。结果:中药联合组胸、腹水临床疗效总效率为76.5%、单灌组为52.9%(P<0.05);治疗前两组患者KPS评分无差异,治疗后两组KPS评分较治疗前均提高(P均<0.05),且中药联合组优于单灌组(P<0.05);中药联合组、单灌组中医证候疗效总有效率分别为82.4%、67.7%,(P<0.05);其中治疗前两组主要症状积分无差异,治疗后两组主要证候积分较治疗前均提高(P均<0.05),且中药联合组优于单灌组(P<0.05);两组患者的不良反应少,差异无统计学意义(P>0.05)。结论:健脾消饮方联合恩度灌注治疗可提高恶性胸腹水患者临床疗效,可改善临床症状,并生活质量得到提高。

Objective:To observe the clinical effect of Jianpi Xiaoyin decoction(健脾消饮方) combined with Endol in the treatment of malignant pleural and ascites.Methods:Sixty-eight patients with malignant pleural and ascites were randomly divided into two groups:the combined group and the single perfusion group.34 cases in each group were treated with Endol infusion.The combined group was treated with Jianpi Xiaoyin decoction(健脾消饮方) on the basis of the treatment of the control group.After treatment,the clinical efficacy,quality of life,TCM syndromes,main TCM symptom scores and adverse reactions were evaluated in the two groups of patients with hydrothorax and ascites.Results:The total effective rate of chest and ascites in Chinese medicine combined group was 76.5% and 52.9% in single irrigation group (P < 0.05);but there was no difference in KPS score between the two groups before treatment. After treatment,the KPS scores in both groups were higher than those before treatment (P < 0.05),and the traditional Chinese medicine combined group was better than the single irrigation group (P < 0.05),and the total effective rate of TCM symptom in the traditional Chinese medicine combined group and single irrigation group was 82.4% and 67.7% respectively (P < 0.05),and there was no difference in the main symptom scores between the two groups before treatment.After treatment,the scores of major symptoms in the two groups were higher than those before treatment (P < 0.05),and the scores in the combination group were better than those in the single group (P < 0.05), but there was no significant difference between the two groups in adverse reactions (P > 0.05).Conclusion:Jianpi Xiaoyin decoction(健脾消饮方) combined with Endol perfusion therapy could improve the clinical efficacy of patients with malignant pleural and ascites,relieve the clinical symptoms,and improve the quality of life.
