摘要
目的探究在膝关节软骨损伤诊断中应用MRI三维扰相梯度回波序列诊断效果。方法选取2016年12月~2018年5月本院收治的疑似膝关节软骨损伤患者206例开展研究,所有患者均经关节镜病理诊断进行确诊,在确诊前所有患者均实施MRI三维扰相梯度回波序列诊断,并以关节镜病理诊断作为金标准,分析MRI三维扰相梯度回波序列诊断准确率、敏感度、特异性。结果 206例患者经关节镜病理诊断出膝关节软骨损伤患者104例;T1WI序列诊断出膝关节软骨损伤患者79例;T2WI序列诊断出膝关节软骨损伤患者84例;WFS诊断出膝关节软骨损伤患者90例;MRI三维扰相梯度回波序列诊断出膝关节软骨损伤患者101例。诊断符合率相比:关节镜病理诊断、MRI T1WI序列诊断、MRI T2WI序列诊断、WFS诊断、MRI三维扰相梯度回波序列诊断符合率相比,差异有统计学意义(P <0.05);且关节镜病理诊断符合率、MRI三维扰相梯度回波序列诊断符合率相比,差异无统计学意义(P> 0.05)。MRI三维扰相梯度回波序列诊断敏感度98.1%;特异度99.0%。结论膝关节软骨损伤采用RI三维扰相梯度回波序列诊断,可提高诊断符合率,且诊断敏感度较高,可为临床提供有效的参考依据,值得临床推广应用。
Objective To explore the diagnostic effect of MRI three-dimensional spoiled gradient echo sequence in diagnosis of knee joint cartilage injury. Methods 206 patients with suspected knee joint cartilage injury who were admitted to our hospital from December 2016 to May 2018 were selected.All patients were diagnosed by arthroscopic pathology.Before diagnosis,all patients were diagnosed by MRI 3 Dspoiled gradient echo sequence.The diagnostic accuracy,sensitivity and specificity of MRI three dimensional spoiled gradient echo sequence were analyzed with arthroscopic pathological diagnosis as the gold standard. Results In 206 patients,104 patients were diagnosed as knee joint cartilage injury by arthroscopic pathology.79 patients with knee joint cartilage injury were diagnosed by T1 WI sequence,84 patients with knee joint cartilage injury were diagnosed by T2 WI sequence,90 patients with knee joint cartilage injury were diagnosed by WFS and 101 patients with knee joint cartilage injury were diagnosed by MRI three dimensional spoiled gradient echo sequence.There were statistically significant differences in the diagnostic coincidence rate of arthroscopic pathology,MRI T1 WIsequence,MRI T2 WI sequence,WFS and MRI three-dimensional spoiled gradient echo sequence(P < 0.05).There was no statistically significant difference in the diagnostic coincidence rate of arthroscopic pathology and MRI three-dimensional spoiled gradient echo sequence(P > 0.05).The diagnostic sensitivity and specificity of MRI three-dimensional spoiled gradient echo sequence were respectively 98.1% and 99.0%. Conclusion MRI three-dimensional spoiled gradient echo sequence in diagnosis of knee joint cartilage injury can improve the compliance rate of diagnosis and it has high diagnostic sensitivity.It can provide an effective reference for clinical practice and is worthy of clinical promotion and application.
