糖化血红蛋白在急性脑梗死诊断和预后中的作用分析 被引量:1

摘要 目的研究糖化血红蛋白与急性脑梗死的相关性。方法回顾我院2016年1月~2017年1月间收治的96例住院患者,其中44例患者出现血糖和糖化血红蛋白升高,为升高组,52例血糖正常,为正常组。对血糖升高组及糖化血红蛋白升高组与正常梗塞灶大小进行比较,分析血糖升高组与正常组预后的关系。结果血糖升高组平均血糖8.371mmol/L,糖化血红蛋白升高组的平均糖化血红蛋白11.59%。梗塞灶面积与正常组相比,血糖升高组和糖化血红蛋白升高组显著增大,差异有统计学意义(F=5.521,P=0.003)。升高组好转率高于正常组,死亡率低于正常组,差异有统计学意义(P <0.05)。有效率和无效率相比较,升高组分别高于和低于对照组,差异有统计学意义(P <0.05)。正常组的有效率高于糖化血红蛋白升高组,高于血糖升高组,无效率低于糖化血红蛋白升高组,低于血糖升高组,差异均有统计学意义(P <0.05)。血糖水平越高,则治疗有效率越低,死亡率越高,血糖水平和死亡率之间呈现正相关性。结论血糖水平高是引起糖尿病后梗死预后效果不佳的重要因素。 Objective To study the correlation between glycosylated hemoglobin and acute cerebral infarction.Methods 96 hospitalized patients who were admitted to our hospital from January 2016 to January 2017 were retrospectively reviewed.Among them,44 patients had elevated blood glucose and glycosylated hemoglobin and they were set as elevated group.52 patients had normal blood glucose and they were set as the normal group.The size of infarct focus in the elevated blood glucose group and the elevated glycosylated hemoglobin group were compared with that in the normal group,and the relationship between the prognosis of the elevated blood glucose group and the normal group was analyzed. Results The average blood sugar was 8.371 mmol/L in the hyperglycemic group and 11.59% in the hyperglycemic group.Compared with the normal group,the infarct area increased significantly in the hyperglycemic group and the hyperglycemic hemoglobin group(F=5.521,P=0.003).The improvement rate of the elevated group was higher than that of the control group,and the mortality rate was lower than that of the control group(P < 0.05).Compared with the control group,the efficiency and inefficiency of the elevated group were higher and lower respectively,and the difference was statistically significant(P < 0.05).The effective rate of the normal group was higher than that of the glycosylated hemoglobin group,higher than that of the glycosylated hemoglobin group,lower than that of the glycosylated hemoglobin group and lower than that of the glycosylated hemoglobin elevation group,with statistical significance(P < 0.05).The higher the blood sugar level,the lower the effective rate of treatment and the higher the mortality rate.There is a positive correlation between blood sugar level and mortality rate. Conclusion High blood glucose level is an important factor leading to poor prognosis of patients with infarction after diabetes.
