开颅减压结合脑室外引流术对高血压脑出血破入脑室感染及预后的影响

Effect of Craniotomy and Decompression Combined with Extraventricular Drainage on Cerebral Ventricular Infection and Prognosis of Hypertensive Intracerebral Hemorrhage

目的探究开颅减压结合脑室外引流术对高血压脑出血破入脑室感染及预后的影响。方法选取2016年1月~2018年10月我院收治的高血压脑出血破入脑室患者46例为研究对象,按照随机数字表法分为对照组和研究组,每组23例。对照组行脑室外引流术,研究组行开颅减压结合脑室外引流术,比较两组治疗效果、感染发生率以及总体预后效果。结果研究组术后出血率、术后感染率低于对照组[(4.35%vs26.09%)、(26.09%vs56.52%)],差异有统计学意义(P<0.05)。研究组预后良好率为69.57%,高于对照组的43.48%,差异有统计学意义(P<0.05)。结论开颅减压结合脑室外引流术能有效清除脑部积血,减少术后再出血和并发症,有效提高了预后效果,具有较高的临床价值。

Objective To investigate the effect of craniotomy and decompression combined with extraventricular drainage on cerebral ventricular infection and prognosis of hypertensive intracerebral hemorrhage. Methods 36 patients with hypertensive cerebral hemorrhage admitted to our hospital from January 2016 to October 2018 were enrolled in the study. The patients were divided into the control group and the study group according to the random number table method, with 18 cases in each group. The control group underwent extraventricular drainage. The study group underwent craniotomy and decompression combined with extraventricular drainage. The treatment effect, infection rate and overall prognosis were compared between the two groups. Results The postoperative bleeding rate and postoperative infection rate of the study group were lower than those of the control group [(4.35% vs 26.09%)、(26.09% vs 56.52%)],the difference was statistically significant (P<0.05). The prognosis of the study group was 69.57%, which was higher than that of the control group 43.48%,the difference was statistically significant (P<0.05). Conclusion Craniotomy and decompression combined with extraventricular drainage can effectively remove blood in the brain, reduce postoperative rebleeding and complications, effectively improve the prognosis, and have high clinical value.
