超声造影鉴别诊断乳腺良恶性病灶的价值 被引量:5

摘要 目的探讨超声造影在乳腺良恶性病灶鉴别诊断中的应用价值。方法对172例乳腺肿块患者行彩色多普勒超声检查,观察病灶大小、形态、内部结构及血流情况,再行超声造影观察病灶超声造影特征,判断其良恶性,并与病理结果对比分析。结果超声造影检出乳腺恶性病灶84例,良性病灶88例,误诊4例,漏诊3例,诊断敏感性95.24%,特异性96.59%。超声造影示乳腺良恶性病灶间增强模式、造影剂分布形式、增强时病灶边界情况、增强强度及增强后范围等方面比较差异均有统计学意义(均P<0.001)。结论超声造影对乳腺良恶性病灶的鉴别诊断有重要价值,值得临床推广应用。 Objective To evaluate the value of contrast-enhanced ultrasound in differential diagnosis of benign and malignant breast lesions.Methods A total of 172 patients with breast masses were examined by color Doppler ultrasound to observe the size,shape,internal echo and blood perfusion of the lesions. Then the contrast-enhanced ultrasound(CEUS) was performed to observe the features of the lesions and to judge its benign and malignant. The results of CEUS were compared with the pathological results.Results There were 84 cases of malignant breast lesions and 88 cases of benign lesions were detected by CEUS,4 cases were misdiagnosed and 3 cases were missed diagnosed. The diagnostic sensitivity and specificity were 95.24% and 96.59%,respectively.CEUS showed significant differences in enhancement pattern,distribution pattern,boundary condition, enhanced intensity and range between benign and malignant breast lesions(all P<0.001).Conclusion The characteristic of CEUS is of great value in differential diagnosis of benign and malignant breast lesions,which is worthy of clinical application.
