摘要
目的探讨长链非编码RNA(lncRNA)H19、ATB与UCA1在宫颈癌的临床价值。方法选取2016年8月至2018年4月在该院经病理学/细胞学确诊的宫颈癌患者130例,同期收取130例宫颈炎患者和130例该院体检健康女性。采用t检验分析血清LncRNA-H19、LncRNA-ATB及LncRNA-UCA1的组间的差异;运用受试者工作特征曲线(ROC曲线)分析血清LncRNA-H19、LncRNA-ATB和LncRNA-UCA1在鉴别诊断宫颈癌的价值;二元Logistic回归分析计算风险比(OR)及其95%置信区间(CI)。结果与宫颈炎患者和体检健康者相比,宫颈癌患者血清LncRNA-H19、LncRNA-ATB和LncRNA-UCA1水平显著增高,差异有统计学意义(P<0.05)。血清LncRNA-H19、LncRNA-ATB和LncRNA-UCA1表达水平与宫颈癌患者发生淋巴结转移(LncRNA-H19:r=0.521,P=0.006;LncRNA-ATB:r=0.505,P=0.011;LncRNA-UCA1:r=0.486,P=0.023)及患者TNM分级(LncRNA-H19:r=0.513,P=0.016;LncRNA-ATB:r=0.591,P=0.003;LncRNA-UCA1:r=0.509,P=0.007)呈线性关联。ROC曲线分析显示,血清LncRNA-H19、LncRNA-ATB、LncRNA-UCA1在区分宫颈癌与宫颈炎和健康对照的灵敏度/特异度分别为:78.5%/86.9%、78.7%/90.7%和70.2%/59.3%;而联合三者后在区分宫颈癌与宫颈炎和健康对照的灵敏度显著提高,达到92.3%。多因素分析显示,在校正年龄、体质量指数、吸烟、饮酒等因素后,血清高LncRNA-H19、高LncRNA-ATB和高LncRNA-UCA1水平仍是宫颈癌发病的独立危险因素(P<0.05)。结论血清LncRNA-H19、LncRNA-ATB和LncRNA-UCA1在宫颈癌中的诊断价值以及在评估宫颈癌患者发生肿瘤远处转移中均具有重要的临床意义。
Objective To investigate the clinical value of long noncoding RNA (lncRNA) H19,ATB and UCA1 in cervical cancer. Methods From August 2016 to April 2018,130 patients with cervical cancer confirmed by pathology or cytology were selected.130 patients with cervicitis and 130 healthy women were enrolled in the same period.The differences of serum LncRNA-H19,LncRNA-ATB and LncRNA-UCA1 were analyzed by t test,and the clinical value of LncRNA-H19,LncRNA-ATB and LncRNA-UCA1 in the differential diagnosis of cervical cancer was analyzed by receiver operating characteristic curve (ROC) technique.The odds ratio (OR) and its 95% confidence interval (CI) were calculated by binary logistic regression analysis. Results The serum levels of LncRNA-H19,LncRNA-ATB and LncRNA-UCA1 in patients with cervical cancer were significantly higher than those in patients with cervicitis and healthy controls,and the differences were statistically significant (P<0.05).Serum levels of LncRNA-H19,LncRNA-ATB and LncRNA-UCA1 in cervical cancer patients have linear correlation with lymph node metastasis (LncRNA-H19: r=0.521, P= 0.006;LncRNA-ATB: r=0.505, P=0.011;LncRNA-UCA1: r=0.486, P=0.023) and TNM grad (LncRNA-H19: r=0.513, P=0.016;LncRNA-ATB: r=0.591, P=0.003;LncRNA-UCA1: r=0.509, P= 0.007).ROC curve analysis showed that the sensitivity/specificity of serum LncRNA-H19,LncRNA-ATB and LncRNA-UCA1 were 78.5%/86.9%,78.7%/90.7% and 70.2%/59.3% respectively in differentiating cervical cancer from cervicitis and healthy controls,while the sensitivity of the combination of the three was significantly improved to 92.3% in differentiating cervical cancer from cervicitis and healthy controls.Multivariate logistic regression analysis showed that high LncRNA-H19,high LncRNA-ATB and high LncRNA-UCA1 levels were still independent risk factors for cervical cancer after adjusting for age,body mass index,smoking and drinking (P<0.05). Conclusion Serum LncRNA-H19,LncRNA-ATB and LncRNA-UCA1 have important clinical significance in the diagnosis of cervical cancer and in the evaluation of distant metastasis of cervical cancer.
