摘要
目的分析640层螺旋CT检查对病理性椎体骨折患者确诊率的影响。方法选取2016年1月至2018年12月我院收治的121例疑似病理性椎体骨折患者,均行X线检查、640层螺旋CT检查,统计X线、640层螺旋CT检出病理性椎体骨折情况,并以穿刺活检、手术病理组织学诊断结果为准,比较X线、640层螺旋CT诊断病理性椎体骨折灵敏度、确诊率、特异度。结果121例患者,经穿刺活检、手术病理组织学证实病理性骨折60例,单纯椎体压缩性骨折61例;X线检出病理性椎体骨折58例,640层螺旋CT检出病理性椎体骨折59例,640层螺旋CT检查灵敏度、确诊率、特异度分别为95.0%、95.9%、96.7%,高于X线检查80.0%、81.8%、83.6%,差异具有统计学意义(P<0.05)。结论640层螺旋CT检查具有覆盖范围宽、扫描速度快、安全性高等优点,诊断病理性椎体骨折能提高确诊率、灵敏度及特异度,应用价值较高。
Objective To analyze the effect of 640-multislice computed tomography(640-MSCT)examination on the diagnosis rate of patients with pathological vertebral fractures.Methods A total of 121 patients with suspected pathological vertebral fractures admitted to our hospital from January 2016 to December 2018 were enrolled.X-ray examination and 640-MSCT examination were performed.The pathological vertebral fractures were detected by X-ray and640-MSCT.The situation was based on the results of needle biopsy and surgical histopathological diagnosis.The sensitivity,diagnosis rate and specificity of pathological vertebral fractures were compared by X-ray and 640-MSCT.Results One hundred and twenty-one patients underwent biopsy and surgical histopathology to confirm pathological fractures in 60 cases,simple vertebral compression fractures in 61 cases,X-ray detection of pathological vertebral fractures in 58 cases,and 640-MSCT detected pathological vertebral fractures.In 59 cases,the sensitivity,diagnosis rate and specificity of 640-MSCT examination were 95.0%,95.9%,and 96.7%,respectively,which were higher than X-ray examination by 80.0%,81.8%,and 83.6%(P<0.05).Conclusion The 640-MSCT examination has the advantages of wide coverage,fast scanning speed and high safety.Diagnosis of pathological vertebral fracture can improve the diagnosis rate,sensitivity and specificity,and the application value is higher.
