骨科手术后切口不愈合的调查与分析

Investigation and Analysis of Incision Nonunion After Orthopaedic Surgery

目的:探讨我院骨科手术患者在术后出现切口不愈合的原因,并找到预防切口不愈合的措施,以减少骨科手术并发症的发生病例,提高手术的成功率。方法:回顾性分析2017年9月至2018年9月期间前来我院接受骨科手术治疗的随机选择的100例患者,对患者的切口愈合情况、伤后至手术时间间隔、手术时长、术后24h切口引流量等因素进行统计学分析,并比较组间差异有无统计学意义。结果:100例骨科手术患者中,共有25例病人出现切口不愈合的现象,不愈合率为25%,切口渗液10例(40%),血肿7例(28%),切口裂开4例(16%),其他4例(16%)。伤后至手术时间间隔、手术时长、术后24h切口引流量,组间差异均有统计学意义(P<0.05)。结论:伤后至手术时间间隔、手术时长、术后24h切口引流量均为我院患者切口不愈合的原因,预防切口不愈合的关键是术前适时把握手术时机,术中熟练操作,术后及时护理。

Objective:To investigate the causes of nonunion of incision in patients with orthopedic surgery in our hospital,and to find measures to prevent nonunion of incision,to reduce the incidence of complications in orthopedic surgery and improve the success rate of surgery.Methods:A retrospective analysis of 100 randomly selected patients who underwent orthopedic surgery in our hospital from September 2017 to September 2018.The factors such as wound healing,time interval from injury to operation,length of operation and 24 hours of incision drainage after operation were statistically analyzed.There was no significant difference between the two groups.Result:Among 100 patients with orthopedic surgery,25 patients had nonunion,25%nonunion rate,10 cases(40%)with incision exudate,7 cases(28%)with hematoma,and 4 cases with incision dehiscence.(16%),the other 4 cases(16%).There were significant differences in post-injury to operative time interval,duration of operation,and 24-hour postoperative incision volume(P<0.05).Conclusion:post-injury to operation time interval,length of operation,and 24-hour postoperative incision drainage are the causes of non-union incision in our hospital.The key to prevent non-union of incision is to grasp the timing of surgery before operation and be skilled in operation,timely care after surgery.
