Rouviere 沟为导向的胆囊三角解剖法在困难腹腔镜胆囊切除术中的应用

The value of Rouviere’s sulcus-oriented anatomy of the gallbladder triangle in laparoscopic cholecystectomy

目的探讨Rouviere沟为导向的胆囊三角区解剖方法在困难腹腔镜胆囊切除术中的应用安全性及疗效.方法回顾性分析笔者2015年1月~ 2018年10月收治的笔者所在医院接受手术治疗的急性胆囊炎患者80例,按所施行的手术方式分为对照组和观察组,每组40例;对照组患者入院后急诊行常规腹腔镜胆囊切除术,研究组患者入院后急诊以Rouviere沟为导向的胆囊三角区解剖腹腔镜下胆囊切除术;比较两组手术方式病人手术时间、胆管损伤、胆漏、出血等并发症及住院日情况等;结果研究组的手术时间、术后住院时间均明显短于对照组(P<0.05),胆管损伤、胆漏也明显少于对照组(P<0.05).结论 Rouviere沟为导向的胆囊三角区解剖方法的腹腔镜胆囊切除术能明显改善急性胆囊炎患者的术中情况,降低术后并发症发生率,是安全可靠易掌握的手术方法.

Objective To investigate the Rouviere groove oriented gallbladder triangle dissection method in hard the application method of safety and efficacy of laparoscopic cholecystectomy were retrospectively analyzed, the author in October 2015 and January 2015 admitted of the authors hospital and surgical treatment of 80 cases of patients with acute cholecystitis, by way of surgery were divided into control group and observation group, 40 cases in each group. Patients in the control group were treated with conventional laparoscopic cholecystectomy in emergency after admission, while patients in the observation group were treated with anatomic laparoscopic cholecystectomy in the triangle region of the gallbladder guided by Rouviere groove in emergency after admission. Postoperative gastrointestinal function recovery and complications were compared between the two groups. Observation group the results of the operation time of postoperative anal exhaust time of postoperative hospital stay white blood cells for the first time back to normal time were significantly shorter than the control group ( P < 0. 05), intraoperative blood loss is significantly less than the control group ( P < 0. 05 ), total complication rates significantly lower than the control group ( P < 0. 05 ) conclusion Rouviere groove oriented gallbladder triangle anatomy method of laparoscopic cholecystectomy can obviously improve the intraoperative situation of patients with acute cholecystitis ,reduce the incidence of postoperative complications, safe and reliable is easy to master the operation method.
