利益相关者视角下的紧密型县乡村医疗服务一体化模式——基于云南省临沧市云县经验

Close-type rural medical service integration model from the perspective of stakeholders —— An experience of Yun County, Lincang City, Yunnan Province

推动医疗工作重心下移、资源下沉,解决人民群众"看病难"、"看病贵"的问题是公立医院改革的主要任务。为此,我国各地开展了医联体的建设,但模式、进展以及成效不尽相同。其中云南省临沧市云县实施的紧密型县乡村医疗服务一体化管理取得了良好的成效。在云县的紧密型县乡村医共体中,充分考虑了各利益相关者的利益需求,从而将各方利益与医共体的发展建设有效整合,使医疗机构的发展以及分级诊疗的落实富有成效。文章以云县的紧密型县乡村医疗服务一体化为例,结合利益相关者理论,分析各类措施对于各方利益需求情况的影响,总结经验,为其他地区建设县域医共体提供借鉴。

It is the main task for the reform of public hospitals to promote the shift of the focus of health care and the shift of resources to primary medical institutions, and to solve the problems of difficulties in seeing a doctor and high cost of getting medical treatment. Therefore, medical partnerships have been encouraged and carried out all over China, but the mode, development and effectiveness are quite different. Among them, the management of close-type rural medical service integration model implemented in Yun County, Lincang City, Yunnan Province has achieved good results. In close-type medical partnerships of Yun County, the interests and needs of various stakeholders are fully taken into account, so as to effectively integrate the interests of all parties with the development of medical community, so as to make the development of health institutions and the implementation of hierarchical diagnosis and treatment fruitful. Taking Yun County’s health service integration as an example, and based on stakeholder theory, this paper analyzes the influence of various measures on the demand of all parties for their interests, summarizes the experience, and provides reference for the construction of county medical communities in other regions.
