摘要
目的 症状性良性前列腺增生(BPH)导致的一系列下尿路症状(LUTS)及其伴发的相关事件,不仅会影响男性患者,还会对患者配偶或女性伴侣产生重要影响,本研究旨在调查症状性良性前列腺增生对患者本人及其配偶生活的影响并进行对比,包括生活质量、日常生活习惯和家庭社交关系。方法 选取255名50岁以上临床诊断BPH伴有LUTS患者及其配偶进行研究。男性患者填写国际前列腺症状评分(IPSS),并要求男性患者及其配偶对睡眠、夜间醒来、性生活、旅游、娱乐、家庭和社交关系的影响填写相关问卷,然后进行统计学分析。结果 BPH患者伴有的LUTS对其本人和配偶的生活均产生程度相近的不良影响。研究发现,绝大多数(88%)接受调查女性的生活因其配偶LUTS而受到了重要影响,包括日常生活、生活质量和家庭社交关系,其中夜间睡眠障碍是最主要的影响(48%),其他包括性生活问题(38%);无法共同长途旅行(19%);无法共同前往没有厕所的地方(13%);无法共同前往电影院或剧院(9%);家庭关系破坏(12%),交际关系破坏(18%)。男性有无LUTS对其配偶夜间起夜、性生活障碍以及无法长途旅行等方面的影响差异具有统计学意义(P<0.05)。结论 BPH伴有的LUTS对男性患者及其配偶均产生了不良影响,在考虑治疗方案时应将女性作为整体的一部分纳入干预范畴。
Objective Symptomatic benign prostatic hyperplasia (BPH) causes a series of lower urinary tract symptoms (LUTS) and its associated events that not only affect male patients, but also have a significant effect on their spouses or female partners. This clinical study aimed to investigate and compare the effects of symptomatic benign prostatic hyperplasia on the patients themselves and their spouses, including quality of life, daily living habits and family social relationships. Methods A total of 255 BPH patients with LUTS over 50 years old and their spouses were all recruited in the study. Male patients filled out the International Prostate Symptom Score (IPSS). Both the patients and their spouses filled out relevant questionnaires on sleep,wake up at night,sex,travel, entertainment,family and social relationships. Statistical analysis was performed for comparison of the effects of symptomatic benign prostatic hyperplasia on life quality of the patients themselves and their spouses. Results LUTS of male patients significantly affected life qualities of their spouses (88%) such as daily life, quality of life, and family social relationships . Among these, sleep disturbance at night (48%) was the most important one, and other issues included sexual problems (38%), long-distance travel (19%), access to places without toilets (13%),inability to go to cinemas or theatre together (9%) family relationship disrupted (12%) and communication relationships destroyed (18%). There was a significant difference in spouses' frequent night-time rises, sexual dysfunction and inability to travel long distances between male patients with LUTS and male patients without LUTS (P<0.05). Conclusion LUTS has adverse effects on the life quality of male patients and their spouses, therefore, patient's spouse should be included as part of the intervention when considering treatment options.
