全髋和半髋关节置换术治疗老年股骨颈骨折疗效分析

Analysis of the Curative Effect of Total Hip and Half hip Replacement for Femoral Neck Fracture in Senile Patients

目的:分析研讨全髋和半髋关节置换术治疗老年股骨颈骨折的疗效.方法:随机从我院2012年1月-2015年12月期间收治的老年股骨颈骨折患者中抽取80例做回顾分析,依据其治疗方式分组,半髋关节置换术治疗的40例为对照组,全髋关节置换术治疗的40例为观察组,观察比较2组患者治疗效果.结果:观察组并发症总发生率2.50%,低于对照组20%,组间数据有统计学意义(P<0.05).观察组术中出血量、总引流量、拔管时间、完全负重时间均低于对照组,对照组手术时间低于观察组,组间数据有统计学意义(P<0.05),比较住院时间,组间数据无统计学意义(P>0.05).观察组Harris评分、假体髋臼外展角、假体髋臼前倾角均优于对照组,组间数据有统计学意义(P<0.05).结论:建议临床治疗老年股骨颈骨折采用全髋关节置换术,有利于恢复髋关节功能,进而提升疗效,值得推广.

Objective: To analyze the effect of total hip and half hip replacement for the treatment of femoral neck fracture in the elderly. Methods: A retrospective analysis was performed on 80 elderly patients with femoral neck fracture who were treated in our hospital from January 2012 to December 2015. 40 patients treated with half hip replacement were in the control group and 40 patients treated with total hip replacement were in the study group. Results: The total incidence of complications in the study group was 2. 50% lower than that in the control group (20%), and the intergroup data were statistically significant (P <0. 05). The intraoperative blood loss, total drainage volume, extubation time, and total weight - bearing time in the study group were all lower than those in the control group, and the operative time in the control group was lower than that in the study group. The data between the groups were statistically significant (P <0. 05). Harris score, acetabular abduction Angle and anterior acetabular Angle in the study group were better than those in the control group, and the data between the groups were statistically significant ( P <0. 05 ). Conclusion: Total hip replacement is recommended for the treatment of senile femoral neck fractures, which is beneficial to the recovery of hip function and the improvement of curative effect.
