损伤控制外科技术对严重腹部创伤急救患者临床结局的影响研究

Effect of Injury Control Surgery on the Clinical Outcome of Patients with Severe Abdominal Trauma

目的:分析严重腹部创伤急救患者应用损伤控制外科技术的效果及临床结局.方法:选择我院2016年2月-2018年6月诊治的88例严重腹部创伤急救患者,随机分成观察组44例与对照组44例.对照组使用常规确定性手术治疗,观察组实施损伤控制外科技术治疗,比较2组临床效果.结果:观察组术中出血量(295.34±34.53)ml、手术时间(64.72±7.63)分钟、住院时间(15.64±2.86)天,优于对照组(P<0.05);观察组CPS(2.78±0.86)分、APS(7.53±2.26)分、APACHEⅡ总分(11.16±3.21)分,优于对照组(P<0.05);观察组并发症发生率15.91%、死亡率6.82%,低于对照组(P<0.05).结论:损伤控制外科技术应用于严重腹部创伤急救患者中的效果显著,可缩短手术时间,减少出血量,减轻应激反应,降低并发症的发生,减少死亡率.

Objective: To analyze the effect and clinical outcome of trauma control surgery in patients with severe abdominal trauma. Methods: 88 patients with severe abdominal trauma treated in our hospital from February 2016 to June 2018 were randomly divided into study group (44 cases) and control group (44 cases). The control group was treated with conventional deterministic surgery, the study group was treated with injury control surgery, and the clinical effects were compared between the two groups. Results: In the study group, intraoperative bleeding was (295. 34 ±34. 53) ml, operative time was (64. 72 ±7.63) min, hospital stay was (15. 64 ±2) min. The total score of Apache U in the study group was ( 11. 16 ±3. 21 ), the incidence of complications in the study group was 15. 91 and the mortality rate was 6. 82, which was lower than that in the control group (P < 0. 05), and the total score of Apache U was (11. 16 ± 3. 21) in the study group (P < 0. 05), and the incidence of complications in the study group was 15. 91%( P < 0. 05), with a mortality of 6. 82B ( P < 0. 05 ). Conclusion: The technique of injury control surgery is effective in treating patients with severe abdominal trauma. It can shorten the operation time, reduce the amount of bleeding, reduce the stress reaction, reduce the occurrence of complications and reduce the mortality.
