造口师联合家庭医生团队模式参与社区压疮管理的研究

Enterostomal therapist combined with family doctor contract service model in community pressure ulcer management

目的 探讨造口师联合家庭医生签约服务模式参与社区压疮高危老人管理及提高居民对相关专科知识知晓率。方法 选取2017年6月至2018年6月社区高危压疮老人78例,按照社区干预模式分为签约组40例,对照组38例。自愿接受造口师联合家庭医生签约服务的老人为签约组,接受家庭医生常规服务的老人为对照组,进行6个月健康干预,比较两组压疮发生率、照顾者掌握压疮相关知识及照顾行为的知晓率。结果 干预后,签约组压疮发生率和严重程度明显低于对照组,差异有统计学意义(P<0.05);签约后,签约组压疮知识得分、照护行为得分分别为(16.75±1.82)分、(51.25±1.61)分,明显高于对照组(9.68±3.53)分、(33.16±3.09)分,差异均有统计学意义(均P<0.05)。结论 造口师联合家庭医生签约服务模式可明显提高压疮高危老人照护者掌握压疮相关知识及预防行为,主动参与压疮预防护理,有助于降低社区老人压疮发生风险,提高签约对象对服务的满意度及生活质量,降低压疮防治费用。

Objective To explore the effect of enterostomal therapist(ET)combined with family doctor contracting service model on the prevention of pressure ulcer in elderly patients with high-risk pressure ulcer and the awareness rate of related professional knowledge in community pressure ulcer management.Methods From June,2017 to June,2018,78 elderly patients with the high-risk pressure ulcer were selected,According to the model of community intervention there were 40 case in the contract group and 38 cases in the control group.voluntarily acceptingthe contract service of enterostomal therapist and family doctor in the community were set as a contract group,who were provided only with family doctor service a control group.Both groups were intervened for 6 months.The incidence of pressure ulcer and the awareness rate of pressure ulcer related knowledge of the caregivers and the care practice of the two groups were compared.Results After the intervention,the incidence and severity of pressure ulcers were significantly lower in the contract group than in the control group(P<0.05);the pressure sore knowledge score and care behavior score were significantly higher in the contract group than in the control group[(16.75±1.82)vs.(9.68±3.53)and(51.25±1.61)vs.(33.16±3.09)],with statistical differences(both P<0.05).Conclusions The service mode of enterostomal therapist combined with family doctor can be adopted.It is helpful to reduce the risk of pressure ulcer and take the initiative to participate in the prevention of pressure ulcer.It also improves the satisfaction and quality of life of the signatories and reduces the cost of prevention and treatment of pressure ulcer.
