摘要
目的 探讨超声引导下甲状腺结节细针穿刺活检结合细胞学现场快速评估(ROSE)对甲状腺结节良恶性诊断的临床价值。方法 选取2017年1月至2018年6月在本院行超声引导下甲状腺结节细针穿刺活检检查的患者220位,分为两组,其中现场细胞学组116例,常规细胞学组104例,以手术病理结果或重复穿刺作为甲状腺结节性质诊断的金标准,比较现场细胞学组、常规细胞学组的诊断满意率、穿刺针数、穿刺时间。结果 现场细胞学组与非现场细胞学组的诊断满意率分别为93.1%、91.3%,差异无统计学意义(P>0.05);现场细胞学组与非现场细胞学组首次活检时间分别为(15.82±1.73)min、(10.43±0.97)min,差异有统计学意义(P<0.05);现场细胞学组与非现场细胞学组的穿刺次数分别为(1.8±0.5)次、(2.7±0.6)次,差异有统计学意义(P<0.05)。结论 现场快速评估不能明显提高甲状腺细针穿刺样本取材满意度,但可减少甲状腺结节的穿刺针数,同时避免部分结节因取材不足造成的二次穿刺。
Objective To investigate the value of rapid on-site evaluation in ultrasound-guide fine needle aspiration biopsy of thyroid.Methods Data were collected from 220 thyroid nodules that underwent thyroid USFNAB.They were randomly divided into the two groups,there were 116 cases in on-site cytology group,while there were 104 cases in routine cytology group.The results were compared with the pathological and clinical follow-up findings,which were diagnosed by biopsy and surgical pathology were included in this study.Results The diagnose rate in on-site cytology group and routine cytology group was 93.1%and 91.3%,the diagnose rate of the on-site cytology was highly consistent with conventional cytological diagnosis.The mean procedural time of inspection was(15.82±1.73)and(10.43±0.97)min respectively,there was statistical difference of abovementioned data between the two groups(P<0.05).And the mean needle passes of inspection was(1.8±0.5)and(2.7±0.6),respectively,there was statistical difference of above-mentioned data between the two groups(P<0.05).Conclusion Rapid on-site evaluation for thyroid FNA can't improve diagnostic performance of thyroid nodules,but can reduces the number of needle passes,especially for some thyroid FNA with inadequate specimens.
