复方黄柏液负压滴灌在糖尿病足溃疡感染中的应用及疗效观察

Negative-pressure instillation of Fufang Huangbaiye for diabetic foot infection

目的 探讨复方黄柏液负压滴灌治疗对糖尿病足感染(DFI)创面及相关炎性因子的影响。方法 选择2016年1月至2018年12月在本院住院治疗的糖尿病足患者62例,按随机数字表法将患者分为对照组与观察组,每组31例。观察组采用复方黄柏液负压滴灌治疗,对照组采用生理盐水负压滴灌治疗。比较两组患者创面愈合率、血清炎症因子水平及创面细菌培养结果的变化。结果 治疗后2周、4周,观察组患者创面愈合率均高于对照组[(23.52±2.13)%比(14.93±2.74)%,(41.25±4.46)%比(30.12±3.08)%],均P<0.05;治疗后2周、4周,观察组细菌培养阳性率均低于对照组[19.35%比45.16%,16.13%比41.94%],均P<0.05。结论 复方黄柏液负压滴灌可加速糖尿病足溃疡感染患者的创面愈合,降低炎症因子水平及细菌培养阳性率,安全有效,值得推广和应用。

Objective To investigate the effect of negative-pressure instillation of Fufang Huangbaiye for diabetic foot infection(DFI).Methods 62 patients with DFI hospitalized in our hospital from January,2016 to December,2018 were selected and divided into a control group and an observation group using the random number table method,31 cases for each group.The observation group was treated with negative-pressure instillation of Fufang Huangbaiye,while the control group with negative-pressure instillation of normal saline.The wound healing rate,serum inflammatory cytokine levels,and bacterial culture results were compared between the two groups.Results 2 and 4 weeks after the treatment,the wound healing rates were higher in the observation group than in the control group[(23.52±2.13)%vs.(14.93±2.74)%and(41.25±4.46)%vs.(30.12±3.08)%;both P<0.05];and the positive rates of bacterial culture were lower in the observation group than in the control group[19.35%vs.45.16%and 16.13%vs.41.94%;both P<0.05].Conclusion Negative-pressure instillation of Fufang Huangbaiye for patients with diabetic foot infection can accelerate wound healing,reduce the level of inflammatory factors and the positive rate of bacterial culture,and is safe and effective,so it is worth for promotion and application.
